Job summary
Working in a small practice, you will be a fantastic team player and independent prescriber, supporting the whole practice with:
- structured medication reviews
- patient medication queries
- initiations and medicines reconciliations
- monitoring of high-risk drugs
- responding and advising on patient safety alerts
- Carry out audits and improve prescribing safety within the practice
Main duties of the job
1. Structured Medication Reviews (SMRs)

Review patients with long-term conditions or polypharmacy.

Optimise treatment according to NICE/local guidelines.

Deprescribe unnecessary or unsafe medicines.

Address adherence issues and side effects.
2. Prescribing & Long-Term Condition Management
-
Initiate, adjust, or stop medicines for conditions such as hypertension, diabetes, asthma, COPD, and heart failure.
3. Medicines Safety & Governance
-
Monitor high-risk medicines (e.g., anticoagulants, methotrexate, lithium).

Ensure appropriate blood test monitoring.

Act on MHRA/DSU safety alerts.

Conduct prescribing audits and quality improvement projects.
4. Repeat Prescription Management

Authorise and process repeat prescriptions.

Ensure monitoring requirements are up to date.

Manage medication queries from patients and pharmacies.
5. Acute & Minor Illness Support

Advise GPs and practice nurses on complex medication queries.

Leicester City who are passionate about excellent clinical care. We have 1 GP
partner, a long-term locum GP, Practice Nurse, HCA and support from a social prescriber, care-coordinator and a fantastic admin team. Situated in a bright, modern, purpose-built

within their professional boundaries, working alongside a team of pharmacists
in general practice.
They will improve patients health outcomes and the
efficiency of the primary care team by providing direct, accessible and timely
medicines and expertise including face to face contact with patients.
The post holder will be an integral part of the
general practice team, as well as part of a wider pharmacist network and
multi-disciplinary team. This includes being a conduit of patient medicines
information into and out of hospitals. They will work to optimise medication
issues to improve patient care and safety, and support clinical staff in the
management of patients.
As part of their employment, the post holder will either
hold or will be willing to gain qualifications from an accredited training
pathway, including independent prescribing, equipping the clinical pharmacist
to be able to practise and prescribe safely and effectively in a primary care
setting.
