Job summary

We are seeking an experienced registered nurse with an interest in respiratory disease management . To be responsible for the delivery of practice nursing services in a friendly, supportive and progressive environment. To work as part of a dynamic multidisciplinary team, delivering a high standard of care within their scope of practice to the practice population.

A Welsh speaker is desirable for this role, but not essential.

Main duties of the job

The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the GP Partners and Practice Manager as appropriate, having regard to current training & regulations. Please see the Job description & person specification for further information.

The Practice area of Meddygfa Taf is predominantly rural, a large farming community, Welsh speaking with a practice population of 7700.

Meddygfa Taf has been a training practice for GP Registrars and medical students for over 30 years. We have a strong training and educational ethos; we understand that we learn by example and sharing of good practice. Our clinical team consists of 6 GP Partners, 2 GP Registrars, 3 Practice Nurses, 2 Health Care Assistants, Phlebotomist, Pharmacy Technician & Cluster Pharmacist. We also work very closely with several external healthcare professionals, such as: Physiotherapists, Occupational therapists, Specialist Nurses, a Social Presciber.

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long-term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
