We are seeking a dedicated and compassionate Practice Nurse to join our dynamic team, with responsibilities spanning two well-established sites: St Nicholas Clinic in Nuneaton and Station Street Surgery in Atherstone. Main duties of the job

  • Deliver high-quality nursing care, including chronic disease management, immunizations, wound care, and health promotion.
  • Collaborate closely with GPs, fellow nurses, and other healthcare professionals to provide comprehensive patient care.
  • Conduct thorough patient assessments, develop and implement care plans, and regularly evaluate patient outcomes.
  • Provide support to patients in managing their health conditions, offering guidance on leading healthier lifestyles.
  • Maintain accurate and detailed patient records while ensuring confidentiality is strictly upheld.
  • Actively participate in team meetings and contribute to the continuous improvement of our services.

  • Communication Skills: Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and the healthcare team.
  • IT Proficiency: Competent in using healthcare software and electronic patient records.
  • Teamwork: Ability to work collaboratively within a multidisciplinary team.
  • Flexibility: Willingness to adapt to the varying needs of the practice and its patients.
  • Commitment to Professional Development: A proactive approach to continuous learning and professional growth.

Desirable

  • Experience: Previous experience working as a Practice Nurse or in a primary care setting is highly desirable.
