Main duties of the job

  • Deliver high-quality nursing care, including chronic disease management, immunizations, wound care, and health promotion.
  • Collaborate closely with GPs, fellow nurses, and other healthcare professionals to provide comprehensive patient care.
  • Conduct thorough patient assessments, develop and implement care plans, and regularly evaluate patient outcomes.
  • Provide support to patients in managing their health conditions, offering guidance on leading healthier lifestyles.
  • Maintain accurate and detailed patient records while ensuring confidentiality is strictly upheld.
  • Actively participate in team meetings and contribute to the continuous improvement of our services.

  • Flexibility: Willingness to adapt to the varying needs of the practice and its patients.
