Your Key Responsibilities
Optimising Medicines & Supporting the Practice Team
- Reduce GP workload by taking the lead on medicines-related queries, prescribing reviews, and medication adjustments.
- Drive improvements in prescribing safety and quality, ensuring compliance with best practices and regulatory guidelines.
- Provide expert support to the wider practice team, offering guidance on complex prescribing decisions and medicines optimisation.
- Be a trusted point of contact for patients, ensuring they have access to safe, effective, and appropriate medication.
- Lead on antibiotic stewardship and medicines safety, supporting practice-wide initiatives to enhance patient care.
Clinical Duties
- Medication Reviews Conduct structured medication reviews, particularly for patients with polypharmacy, frailty, or multiple long-term conditions.
- Chronic Disease Management Support patients with hypertension, Type 2 Diabetes, asthma, COPD, and other long-term conditions.
- Medicines Reconciliation Ensure safe, effective transitions of care by reviewing medications when patients move between providers.
- Care Home & Frailty Reviews Work proactively to reduce inappropriate prescribing and optimise medication regimens for vulnerable patients.
Dispensary Leadership & Development
- Oversee and develop the practice dispensary, ensuring it operates efficiently, safely, and in line with regulations.
- Support and manage dispensary staff, providing mentorship, training, and ensuring compliance with audits and best practices.
- Drive quality improvement initiatives, identifying opportunities to enhance efficiency and patient experience in the dispensary.
