Job summary

The Frailty Care Coordinator will work with the PCN team and Community Trust to have a robust and effective service for the Integrated Frailty Teams service. The Patient Care Coordinator (PCC) will be required to deal with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional.

Main duties of the job

This exciting role has various components which include:

  • Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care
  • Support patients to utilise decision aids in preparation for a shared decision-making conversation
  • Provide coordination and navigation for people and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches and other primary care professionals
  • Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients and, where required and as appropriate, refer people back to other health professionals within the PCN

About us

Our PCN consists of 16 practices and includes various roles such as Pharmacists, Pharmacy Technician's, Paramedics, PA's, Health Coaches, Physios and Podiatrists. There is also great support from GPs, nurses and support staff enabling us to become an integral part of the medical team.

Qualifications

Essential

  • Education, Qualifications and Training:
  • NVQ Level 2 or equivalent
  • Willing to work towards NVQ Level 3
  • Experience and Knowledge:
  • Minimum of 2 years in health or social care profession
  • Knowledge of primary care
  • Experience of working in a multi-disciplinary setting
  • Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
  • Experience of administrative duties
  • Skills and Attributes:
  • Proven record of excellent written and verbal communication skills and
  • interpersonal skills
  • Excellent organisational and admin skills
  • Analytical skills and ability to interpret information and present it in a clear and concise manner
  • Evidence of excellent knowledge and competence with Microsoft Office
  • Able to deal with service users sensitively
  • Able to work effectively as part of a team
  • Able to prioritise and manage own workload with minimal supervision
  • Willingness to undergo further training and committed to own development
  • Car user as travel between sites across the Wallasey/Birkenhead area will be required
