Practice Nurse Job Description Premier Health Team.

Role Summary:

  • To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
  • The practice nurse will be responsible in a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

Primary Responsibilities:

  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health.
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE, GMMMT and the BNF.
  • Provide vaccination and travel medicine services.
  • Request pathology services as necessary.
  • Process pathology results as required.
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Ensure read codes are used effectively.
  • Maintain chronic disease registers.
  • Develop, implement, and embed well woman clinics.
  • Develop, implement, and embed NHS Health Check clinics.
  • Implement vaccination programmes for adults and children.
  • Prioritise health issues and intervene appropriately.
  • Recognise, assess, and refer patients presenting with mental health needs.
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice).
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies.
  • Deliver opportunistic health promotion where appropriate.
  • Maintain baby immunisation results logs.
  • Maintain smears results logs.
  • Manage emergency drug kit.

Part time General Practice Nurse Vacancy

Premier Health Team are looking for a part time General Practice Nurse to work 24 hours per week in our innovative and thriving practice. We require a flexible candidate able to work between the hours of 8am-6.30pm who will work alongside our team to continue to help us achieve our exceptional standards of patient care. The hours and salary are negotiable dependent upon individual needs and experience.

Rated as outstanding in our CQC inspection in 2016 and again at interim review in July 2019, we are a practice serving over 4,000 patients in a working town under the umbrella of Greater Manchester Integrated Care Board. We are part of the Leigh Primary Care Network, ensuring we provide the best services for our patients and those within the local area.   • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  • Effectively utilise the audit cycles.
  • Monitor and ensure the safe storage, rotation, and disposal of medicines.
  • Support other members of the nursing team, providing guidance and support when necessary.
  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning within the practice.
  • Continually review clinical practices, responding to national policies and initiatives where appropriate.
  • Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA).
