Women's Health Lead

Cricklewood General Practice are looking for Advanced Nurse Practioner to join our team for 3 days per week, on Wednesdays, Thursdays and Fridays.

This role will be Womens Health Lead for the practice.

We are looking for a candidate who will take an active role in all aspects of womens health, including improving screening uptake for Breast and Cervical screening, conducting Cervical Smears, Coil Fittings and HRT clinics.

Due to the nature of the role, there is an occupational requirement that this is a female only role.

Main duties of the job

This role will provide care for patients presenting with acute, non-acute and chronic medical conditions.

This will involve taking the patient history, conducting physical examinations as appropriate, formulating differential diagnoses and ensuring relevant follow up, treatment plans or next steps are taken.

A major component of the role will be specific womens health clinics to support our local patient population, including cervical smears, coil fittings and HRT clinics.

We currently have a patient population of 5,000 and serve an ethnically diverse urban patient population.

POST TITLE: Advanced Nurse Practioner / Womens Health Lead

RESPONSIBLE TO: Senior Salaried GP

Job responsibilities

Work within the professional Code of Conduct and other directives as defined by the NMC.

Work autonomously and be accountable for own professional actions.

Practice Lead for Womens Health, providing specific patient clinics and working with management team to improve screening uptake within patient population.

Be responsible for remaining up-to-date with CPD requirements to maintain professional registration with the NMC; including revalidation every three years.

Ensure own educational commitment is at least sufficient to maintain CPD requirements and to ensure own mandatory training certificates are current, in line with organisational policy.

Provide clinical leadership, support and training to colleagues and students.

Demonstrate a clear understanding of the accountability and legal implications of the ANP role.

Participate in a regular performance review with the Senior GP, using an agreed competency framework appropriate to the individuals scope of practice.

Clinical Responsibilities

Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.

Assess patients with a range of acute, non-acute, and chronic medical conditions.

Take a history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.

Highlight any gaps in knowledge/training needs to the Senior GP and allow them the opportunity to offer support in order to fulfil the duties of this post.

Remain up-to-date with clinical guidelines to ensure that practice remains evidence based; particularly guidelines for the management of chronic diseases and national/local guidance/ICB formularies when prescribing.

Where appropriate make referrals to other healthcare professionals in primary and secondary care and arrange hospital admission for applicable cases.

Attend MDT meetings and maintain effective relationships with multi-professional colleagues; such as the care co-ordinators, community palliative care team, district nurses, specialist nurses, mental health and social care.

Undertake telephone triage work and provide telephone advice calls/signposting to other services as appropriate.

Undertake consultations in the most appropriate setting, using various consultation types; including, but not limited to, face to face, video, telephone, e-consultation.

Undertake medication reviews in line with local medicines management guidelines and with the support of the locality pharmacist.

Ensure clinical practice is safe and effective and remains within the boundaries of competence.

Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary

Work in line with organisational policies and procedural guidelines.

Attend and participate in practice meetings as required

Take part in clinical audit to continually review and improve the quality-of-care provision.

Prescribe treatment in line with local and national guidelines and ICB formulary.
