Old Swan Health Centre Group Practice is looking to recruit
an experienced, dynamic primary care pharmacist, to join our busy but friendly
team serving 9300 patients. We are
looking for a practice pharmacist to work closely with the team to ensure we
deliver an effective, efficient and quality medicines management service to our
patients and our team.
Main duties of the job
As a Prescribing Pharmacist, you will be
responsible for efficient prescription signing, medication reviews, and
reconciliation, ensuring safe and effective prescribing across the
practice. You'll conduct structured medication reviews, working
closely with patients and clinicians to optimize treatments, reduce risks, and
improve adherence.
A key part of your role will involve running safety
searches and implementing medication changes in line with best
practice, ensuring compliance with the latest guidelines. You'll collaborate
with GPs, nurses, and the wider team to enhance prescribing safety and support
long-term condition management.
This role offers a balance of independent clinical
work and team collaboration, making it ideal for a pharmacist looking to
apply their expertise in a patient-focused setting.

Our team includes 5 partners, 3 salaried GPs, 4 Registrars,
ANP, 2 Practice Nurses, HCA, Physiotherapist, Practice Manager,
Reception Manager, Secretary and a team of Receptionist/Care Navigators and
Administrative Staff.

- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- Experience in primary care, community pharmacy, or hospital settings.

- Ability to work independently while collaborating within a multidisciplinary team.
- Excellent communication and patient consultation skills.
