Job summary

Walnut Tree Surgery has an exciting opportunity to welcome up to two new individuals to join our small and friendly team as part of a Medical Care Coordinator.

Vacancy

Tues 7:30 - 14:00 (5.5 hours)

Weds 13:45 - 18:30 (4.75 hours)

Thurs 7:30 - 14:00 (5.5 hours)

Total = 15.75 hours

The successful applicant will be enthusiastic, welcoming, confident, and self-motivated. The individual will need to thrive in working in a challenging, busy environment and ideally have previous experience of working with the public. The work will mainly be telephone and IT based so you should have an excellent customer service background as well as good keyboard skills and a working knowledge of Microsoft Office packages is desirable. Main duties of the job

You will work within a varied professional group all situated at the Practice, as well as with members of the public. Therefore excellent communication, interpersonal and organisational skills are paramount within this role. You will need to be a team player with flexibility, be able to follow protocols and procedures and use your own initiative, work efficiently in a busy environment, whilst prioritising workload.

You will be responsible for undertaking a wide range of reception/admin duties and the provision of general support to the wider team. Duties can include but are not limited to, greeting and directing patients, effective use of the appointment system, processing of information, and assisting patients and team members as required. This role will involve dealing with a high level of telephone enquiries, face to face contacts and medical administration.

Job responsibilities

For those patients who needs we are not able to meet through the routine navigation process, to support them through the receptions care navigation process.

To ensure timely, efficient, and patient centred care planning for patients and their needs.

To review patients needs and help to connect them to the services and support they require, whether within the practice or elsewhere.

Initiate contact with and respond to requests from patients, team members and external agencies.

Work with individuals in a case management function to develop, implement and review their wellbeing care plan.

To assess the needs of individuals identifying outcomes and determining the type of support required to overcome barriers and achieve outcomes.

To undertake, review and amend risk assessments in relation to activities, clients and the environment.

Working with the social prescriber, signpost to existing community groups within the delivery and wider network, which will support the individual in their achievement of wellbeing goals

Support in regular case conferences with partner organisations, support services and/or other team members to ensure a client journey is smooth and consistent.

Undertake follow up contact to identify whether a patient has maintained their level of well-being and identify any risk to achieving this.

Be fully responsible for all tasks related to journey / case management function including maintaining effective and efficient administrative procedures producing appropriate records and reports, including the updating of client details on the clinical system.

Capture management information from delivery and wider network and provide reports accordingly.

Coordinating clinics, booking appointments, dealing with admin, collating data and reports and providing feedback to the practice.

Direct access for other health professionals to update/or make requests for the most venerable patients

Coordinating Ardens Manager uploads to assist whole PCN

To liaise with palliative care and District Nursing teams to support patients care needs
