Job summary

An exciting opportunity has arisen for a Practice Nurse to join our friendly hardworking team at Griffiths Practice, Ashmore Park, Wednesfield. We are a recently remodelled and refurbished GP Practice, with a new Multidisciplinary Team in place. Griffiths Practice is achieving very high standards in terms of our Quality Framework and we are very proud to have achieved a Good rating by the Care Quality Commission. We are receiving excellent feedback regarding our service delivery, and have a rapidly increasing list size.

We are looking for a highly motivated and committed individual to join our practice. Previous experience of working in Primary Care is preferable although not essential. Full in-house training will be provided with GP and ANP support on site.

We are a happy team working in a supportive and friendly environment, where each staff members contributions are highly valued.

Terms and conditions can be negotiated depending upon skills and experience. You will be responsible for all Practice Nurse duties. The practice has opening hours of Monday to Friday 8am to 6.30pm.

Main duties of the job

All Practice Nurse duties including;

Wound Care/ dressings

Phlebotomy

Immunisations and Other Injections (Hormone, B12, travel and contraceptive)

Cytology - Smear Taking and Swabs

Chronic Disease Monitoring and Reviews (Asthma, Diabetes, Hypertension etc..)

Blood Pressure Monitoring

Diabetic Foot checks and monitoring

Face to face and telephone appointments with patients

Stock Monitoring

Vaccination Monitoring and Audit

Fridge Temperature Monitoring

Infection Control Audits

Urinalysis

Main Purpose

The practice nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of Practice.

Providing assessment, screening and treatment services and health education advice

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care and minor ailments with referral to GPs as necessary.

Supplies and equipment Treatment room and other areas

In participation with the practice manager, ensure the maintenance of equipment and stock levels relating to patient care.

Pathological specimens and investigatory procedures

Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in practice meetings as required.

Assist in the formulation of practice philosophy, strategy and policy and develop appropriate protocols.

Maintain a notice board in the waiting area designated for patient health care education.

Research projects

Co-operate and participate as required in any research projects within the practice

Student/staff education and training

Participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff.

At all times there will be a need to maintain accurate records

A duty to advise GPs/Practice Manager of potential problems or errors within the range of assigned tasks

Attend and participate in any practice meetings when required.

Undertake home visits if required
