Job summary

We are looking for an independent, motivated and forward-thinking Advanced Clinical Practitioner to join our growing same day care Team who undertake the assessment and treatment of patient needs.

We are an established training practice in Swindon. We have a diverse clinical team including a Paramedic, Clinical Pharmacists, First Contact Physios, Admin support, Nurses & HCAs who form a strong, skilled team that are committed to providing high quality care and service.

Main duties of the job

You will work with our fantastic & expanding Team, supported by GPs, a Home Visiting Paramedic, Advance Clinical Practitioner & Clinical Administrators to provide both in practice and home visiting on the day/urgent patient appointments.

The main duties of the role will be patient facing clinics including home visits managing minor ailments and acute conditions. You will also support the clinical team as we move to greater digital access for our patients.

The successful candidate will work closely with the varied wider team of clinicians to provide high quality care to a range of patients and support with meeting QOF and IIF targets, participating in vaccination programmes.

Job description

Job responsibilities

The main duties of the role will be patient facing clinics including home visits managing minor ailments and acute conditions, this will include the following:

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.

Request and undertake the collection of pathological specimens where necessary.

Process, interpret and action pathology and other test results as required

Keep accurate notes on patient care and ensure SNOMED CT codes are used effectively.

Recognise, assess and refer patients presenting with mental health needs

Prioritise health issues and intervene appropriately.

Support the implementation and evaluation of individual specialised treatment plans for chronic disease patients. Promoting the management and support of patients at risk of developing long-term conditions, preventing adverse effects to the patients health.

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.

Contribute to practice targets and the formulation of documents (QOF, etc.), complying with local and regional guidance.

Support with all safeguarding matters in accordance with local and national policies. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

Support the team in dealing with clinical emergencies and digital requests.

Deliver opportunistic health promotion and data collection where appropriate.

Participate in the home visiting service including ad hoc visits to care homes.

Able to refer appropriately to secondary care or organise emergency admissions.

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
