Job summary

An opportunity has arisen for a committed and enthusiastic Practice Nurse to join our team at Downsfield Medical Centre. The post is for approximately 12 hours per a week on a permanent basis. The ideal working day's would be a Wednesday and Thursday however this is negotiable. We are a well-established, GP Practice delivering family - centered continuity of care patients.

Main duties of the job

The post holder is responsible for the delivery of practice nursing services care to our practice population.The post will have a focus upon supporting our patients to be healthy with monitoring of long-term conditions, health prevention and screening activities. The post holder will work collaboratively with the general practice team to meet the needs of our patients in a compassionate way.

Downsfield Medical Centre is a forward thinking training practice of just under 5000 patients. Our warm and friendly surgery is led by one senior GP partners, supported by two salaried GPs and two Advanced Nurse Practitioners. We also have a diverse multi disciplinary team consisting of a practice nurse, pharmacists and nurse associate. The practice is also supported and managed by a strong and experienced team of managers, practice leads, admin and reception staff.

Job responsibilities

General Practice experience in the following areas is desirable:

  • Undertake health screening, surveillance, new-patient screens and immunisation
  • Vaccination and immunisations, administering injections, ensuring safe vaccine storage, implementing cold chain policy
  • Undertake cervical cytology screening on those who are eligible in line with national guidelines
  • To perform venepuncture
  • Assisting with minor surgery
  • Respiratory and other chronic disease management including hypertension, copd management,
  • Cytology
  • Dressings & wound care including removal of sutures
  • Travel vaccinations
  • Baby immunisations
  • Family planning & pill checks
  • Provide direct patient care in a medical practice setting
  • Assist healthcare providers with examinations and procedures
  • Document patient information accurately and maintain electronic medical record
  • Educate patients on preventive care, medication instructions, and lifestyle changes
  • Collaborate with the healthcare team to develop and implement patient care plans
  • Monitor and ensure the safety and comfort of patients during their visit

Other tasks will include

  • Maintaining and cleaning equipment
  • Maintain general tidiness and cleanliness of nurses and treatment room
  • Ordering of vaccinations to maintain stock level
  • Participation in administrative systems in the Practice
  • At all times there will be a need to maintain accurate records
  • Attend and participate in any Practice meetings when required.
  • On occasion there may be a requirement to undertake home visits
  • Involvement in QOF related activity

  • UK registered nurse with significant post registration experience
  • Must have a good knowledge on Quality outcomes Framework
  • Must be able to relay clinical information and maintain accurate, comprehensive consultation records
  • Must have PC skills ideally will be able to use basic Word, Excel and e-mail
  • Able to work in a changing environment and implement change when applicable to provide high quality health outcome
