Job summary

Cambridge Medical Group is looking to recruit a full time Practice Nurse. The practice is looking to employ an enthusiastic nurse who is committed to working alongside our existing nursing team to achieve outstanding patient care.

Main duties of the job

The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, immunisations, cervical screening, wound care and health education. The Practice Nurse will work under the guidance and supervision of our Lead Nurse, contributing to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.

The Practice Nurse is expected to perform clinical procedures and manage patient follow-ups, ensuring the delivery of care is in accordance with the latest clinical guidelines and within the scope of practice. This position requires excellent nursing skills, a patient-centred approach to care, and the ability to work effectively within a multi-disciplinary team to improve health outcomes.

Cambridge Medical Group provides care to 7,400 patients across Middlesbrough and the immediate area. The surgery is made up of a diversified clinical team consisting of four GP Partners, one salaried GP, NP's, and a nursing team, with clinical supervision and mentorship by GPs to GP registrars and F2 doctors.

Job description

Job responsibilities

Clinical Responsibilities

  • Carry our nursing interventions and measurements for patients using clinical judgement.
  • Administer medications by injection, with the appropriate legal authorisation.

  • Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques, which may include brief intervention sign-posting.
  • Undertake initial assessment of patients presenting with wounds, produce treatment plans, and refer to other services as necessary.
  • Measure circulation by recording Ankle Brachial Pressure Index and applying compression therapy to patients with venous leg ulcers.
  • Remove wound closure materials including sutures, clips, and staples.
  • Take samples for the cervical screening programme according to recognised standards.
  • Administer vaccines to adults and children according to the UK immunisation schedule.
  • Administer prescribed therapies such as vitamin B12 and Depo medications, recognising side effects and contraindications.
  • Provide sexual health services.
  • Advise women on the suitability of oral and long-term contraceptive methods and be aware of local referral pathways.

Health Promotion and Education

  • Deliver a range of health care services that support health promotion and prevention including sexual health and participate in public health programmes and initiatives to promote wellness and disease prevention within the community.
  • Deliver educational support to patients on health management, disease prevention, and lifestyle modifications.

Chronic Disease Management

  • Provide a nurse-run asthma / diabetes / COPD service to patients, to include screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidelines and Quality and Outcome Framework indicators.
  • Follow protocols for the management of chronic diseases and chronic disease registers such as diabetes, asthma, COPD, and cardiovascular disease.
  • Regularly review and monitor patient health status and adjust care plans as necessary.

Administrative and Record Keeping

  • Maintain accurate and detailed patient records, ensuring confidentiality and compliance with GDPR.
  • Utilise SystmOne to enter patient data and retrieve patient information efficiently.

Team Collaboration

  • Work closely with GPs, Lead Nurse, and other healthcare professionals to ensure cohesive patient care and service delivery.
  • Engage in continuous professional development to keep up-to-date with the latest healthcare practices and innovations.
  • Facilitate learning experiences of other clinicians, including trainees, provide supervision, and promote shared learning.

Quality Assurance / Service Support

  • Participate in quality improvement and innovations, e.g. audit, significant event analysis, and development of protocols and new services.
  • Recognise opportunities to reduce inequalities and unwarranted variations in healthcare for the practice population.
  • Undertake mentorship for preceptor programmes and contributing to revalidation needs of nurse colleagues.
  • Analyse data and develop initiatives to improve uptake of national screening and immunisation programmes, including coordinating new vaccination campaigns.

  • Complete statutory training / updating and mandatory training as defined by the practice.
