Job summary
Due to retirement an exciting opportunity has arisen for an experienced Practice nurse to join our friendly team. You will undertake various responsibilities such as chronic disease management, health promotion, and clinical procedures.
Main duties of the job
Job Responsibilities:
Deliver practice nursing services within the NMC Code of Conduct and agreed Practice standards.
Manage patients with chronic diseases, following quality frameworks and evidence based guidelines.
Oversee infection control, ensuring compliance, and participate in annual audits.
Perform clinical duties, including chronic disease management, cervical screening, immunisations and lifestyle advice.
Maintain accurate records, participate in audits and update disease registers.
Key Skills:
Ability to work independently and collaboratively within a multidisciplinary team.
Strong organisational and planning skills, able to work under pressure.
Excellent communication skills and experience in dealing with the public/patients.
Willingness to develop nursing skills and adapt to a changing environment.
About us
We are a small practice with over 3,000 patients supported by an experienced clinical and administrative team. We are flexible with the number of hours required for the post but a minimum would be 22.5 hours.

- Registered General Nurse (diploma or degree level), NMC Registration.
- Practice Nurse Qualification.
- Experience in chronic disease management, cytology and immunisations.
Desirable
- Familiarity with Emis Web clinical system.
- Car driver.
