Job summary
As
part of East Cleveland Group Primary
Care Network's multi-disciplinary team, the post-holder will work collaboratively to improve patient outcomes and support the
PCN in achieving the ambitions of the NHS Long Term Plan.
The post holder will work closely with a wider multi-disciplinary team (MDT), PCN member practices across Skelton,
Guisborough, Brotton and Loftus to ensure patients with a cancer diagnosis receive
the best possible care and service.
The role will include communicating
with patients to ensure they understand their condition, care pathway and
treatment and to make sure that they are receiving the most appropriate support.
As a newly established role, the post
will be subject to review.
Main duties of the job
The
Cancer Care Coordinator will work with the PCN central team supporting the
delivery of NHS Englands Comprehensive model of Personalised Care and the
Early Cancer Diagnosis Direct Enhanced Service. This pivotal role will support
multidisciplinary teams and coordinate the pathway for patients with cancer,
supporting public awareness of the earlier diagnosis agenda, as per the NHS
Long Term Plan. The Cancer Care Coordinator will work with patients, ensuring
they have the right support to understand their diagnosis and what the next
steps are to help them during this distressing time, including personalised
care interventions such as Personalised Care and Support Planning (PCSP) and
Cancer Care Reviews.
This
is a patient-facing role, and the post holder will
be responsible for providing support directly to patients and their carers. The
post holder will act as a conduit for cancer patients within the PCN, liaising
with secondary and tertiary care and the voluntary sector and, working closely with practice
clinical leads and cancer champions where appropriate.

signposting to services such as the hospital team, district nursing or
Macmillan services, benefits agency etc as appropriate. To also act as point of
contact for local hospice providing liaison to ensure care planning is in place.
Support
individual practices to improve local uptake of National Cancer Screening
Programmes, understanding practice data for uptake and how screening is carried
out, through community of practice which supports peer to peer learning events,
and engagement with local providers.
Ensure
that Cancer Care Reviews have been undertaken with all patients within 12
months of diagnosis.
In
addition to collaborating with the wider PCN team eg Social Prescribers,
Pharmacists, and other clinical/non-clinical partners involved in the patients
care, the postholder will engage with local system partners, including Patient
Participation Groups, secondary care, Public Health Commissioning teams, Northern
Cancer Alliance and other external stakeholders as appropriate.

Essential
- Previous experience in the NHS, specifically in primary care
- Experience of administrative duties
- Practical experience of being in a patient facing role
- Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
Desirable
- Experience of working in a cancer care setting
- Experience in promoting health and health improvement within the community, including health promotion campaigns and signposting.
Qualifications
Essential
- Educated to GCSE Standard (Grades A-C) or Diploma/ HNC level (or relevant experience)
- ECDL or equivalent IT qualifications and experience of using various systems and packages, eg Microsoft Outlook, Word, Excel & PowerPoint, databases and internet
Desirable
- NVQ Level 2 Business Administration (or relevant experience)
- Long term condition training
- Knowledge and understanding of Primary Care Network arrangements and their contribution to the NHS 10-Year Plan
- Full, clean driving licence
