Job summary

East Cleveland PCN Ltd is seeking a Clinical Pharmacist to join an exciting and forward-thinking primary care network with ambitions to truly transform patient care.

This is a great opportunity for a Pharmacist looking to progress in patient-facing, person-centered, integrated care. Main duties of the job

As part of East Cleveland PCN's expanding pharmacy team, the post-holder will play a key role in developing & maintaining clinically and cost-effective prescribing provision across the network population.

With a passion for person-centred care, the post holder will be responsible for the care management of patients with chronic diseases undertaking clinical medication reviews to support proactive management of complex polypharmacy patients.

The post-holder will also provide clinical leadership in the delivery of person-centred medicines optimisation, supporting standardised approaches to care across the PCN & forging strong relationships with specialist and community pharmacists to promote system-wide medicines optimisation & quality improvement.

The role will include supported progression through the CPPE Primary Care Pharmacy Education Pathway & independent prescriber qualifications - providing an exciting development learning opportunity, alongside rewarding career prospects.

They will support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safe prescribing providing expertise in clinical medicines advice, whilst proactively and reactively addressing both public and social care needs of the network population.

Qualifications

Essential

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

Desirable

  • Hold or be working toward an independent prescribing qualification (although this will be offered as part of the role).
  • Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered as part of the role).

Experience

Essential

  • Minimum 2 years pharmacist experience.

Desirable

  • Experience in conducting medication reviews.
  • Experience in developing and implementing new policies and processes to improve the quality and safety of practice, in the context of medicines optimisation.
  • Experience in supporting common acute and long-term conditions. Experience of working in GP practice.
  • Experience in working in a care home setting in the capacity of providing patient-centred medicines optimisation support.

Knowledge and skills

Essential

  • Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role.
  • Audit and quality improvement, including the review and analysis of data; risk/issue management and addressing quality/performance in practice.
  • Digital skills, including ability to access and competently use clinical systems and electronic transfer of prescriptions.
  • Strong clinical leadership, communication and interpersonal skills.
  • Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role.
