Job summary

East Cleveland PCN Ltd is seeking a qualified Advanced Clinical Practitioner for our Advanced Clinical Review (ACR) Hub.

The ACR Hub will provide a referral pathway for patients requiring acute clinical review and/or follow-up which is delivered through a mixture of face-to-face, telephone and digital media. This innovative service is supported by Duty GPs from member practices. The successful applicant will also be responsible for the development of clinical pathways as the service develops.

Based at one of the PCN core member practices, there may be a requirement to travel to other locations across the Network.

Main duties of the job

As part of East Cleveland's multi-disciplinary team, the post-holder will be qualified to MSc ACP level and be expected to work independently in providing clinical care to adults and children. The successful candidate will assess, diagnose, treat, refer or signpost patients with undifferentiated or undiagnosed conditions relating to minor illness or injury, supported by senior clinicians as required.

Job description

Job responsibilities

  • Be part of the PCN's care provision, working as an autonomous practitioner
  • Consult acute presentations or exacerbations via a combination of telephone triage, face-to-face appointments and home visits
  • Have clinical skills (Adult and Children)
  • Hold an independent prescribing qualification
  • Have experience of Chronic Disease Management
  • Work closely with a wider multidisciplinary team including nurse practitioners, GP's, practice nurses, pharmacists and administrators to triage appointments and home visit requests
  • See patients with minor ailments, either in practice or in their own/care home setting
  • Use advanced clinical skills to provide education to patients and service users, promoting self-care and empowering them to make informed choices about their treatment
  • Assess, diagnose, treat, refer or signpost patients with undifferentiated or undiagnosed conditions relating to minor illness or injury, supported by senior clinicians as required
  • Hold a full driving licence; this is essential as home visits may be a requirement of this role
  • For ACP nurse applicants only: you should have graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's e-portfolio route to enable eligibility for an 'Advanced' digital badge.
