JOB TITLE: Nursing Associate
REPORTS TO: Lead Nurse
Job Summary
We are seeking an enthusiastic, NMC-registered Nursing Associate to join our expanding primary care team. You will work collaboratively within our multi-disciplinary environment to deliver high-quality nursing care that supports the health and wellbeing of our local community of 8,000+ patients. Key Clinical Responsibilities
Chronic Disease Management
- Lead chronic disease clinics for diabetes, asthma, COPD, hypertension, and other long-term conditions
- Conduct comprehensive health assessments and annual reviews in line with national and local guidelines
- Medication management including reviews, reconciliation, and monitoring
- Patient education and self-management support for chronic conditions
- Care planning and goal setting with patients and families
Health Promotion and Prevention
- NHS Health Checks and cardiovascular risk assessments
- Health promotion activities including smoking cessation, weight management, and lifestyle advice
- Vaccination services including seasonal flu, COVID-19, and travel immunizations
- Screening programs including cervical screening, bowel screening coordination
- Preventative care and early intervention strategies
Clinical Assessment and Treatment
- Comprehensive nursing assessments using systematic approaches
- Clinical observations including vital signs, ECGs, and physiological monitoring
- Wound care management including assessment, treatment, and healing monitoring
- Minor illness management within scope of practice and competency
- Clinical procedures as trained and assessed competent
Collaborative Care Delivery
- Multi-disciplinary team working with GPs, Practice Nurses, and allied health professionals
- Care coordination and case management for complex patients
- Referral management to secondary care and community services
- Liaison with district nurses, social services, and external agencies
Professional and Governance Responsibilities
Clinical Governance
- Maintain NMC registration and meet revalidation requirements
- Adhere to clinical protocols and evidence-based practice guidelines
- Participate in clinical audits and quality improvement initiatives
- Incident reporting and learning from adverse events
- Risk assessment and management in clinical practice
Safeguarding and Protection
- Safeguarding awareness for children, young people, and vulnerable adults
- Recognise and respond to safeguarding concerns appropriately
- Documentation and reporting of safeguarding issues
- Multi-agency working in safeguarding cases
Infection Prevention and Control
- Maintain infection control standards in all clinical activities
- Implement IPC protocols including hand hygiene, PPE, and environmental cleaning
- Monitor and audit IPC compliance
- Education and training of other team members in IPC practices
Information Management and Communication
Record Keeping and Documentation
- Accurate clinical record keeping using SystmOne clinical system
- Contemporaneous documentation of all patient interactions
- Clinical coding and data quality management
- Confidentiality and data protection compliance
Patient Communication
- Therapeutic communication with patients and families
- Health education and information provision
- Difficult conversations with sensitivity and professionalism
- Cultural competence in diverse patient populations
Team Communication
- Effective handovers and communication with colleagues
- Participation in team meetings and clinical discussions
- Mentoring and supervision of healthcare assistants and students
- Collaborative problem-solving within the healthcare team
