South Lincolnshire Rural PCN, is a forward-thinking Primary Care Network, consisting of Holbeach Medical Centre, The Suttons Surgeries, Littlebury Medical Centre, Moulton Medical Centre, Abbeyview Medical Centre, Bourne Galletly Practice, Gosberton Medical Centre, Sutterton Medical Centre and The Deepings Practice.

We are seeking an enthusiastic and forward-thinking pharmacist, to join our like-minded PCN. We want to find the best pharmacy talent to join our team. You will become part of a group of PCN Clinical Pharmacists working under the supervision of the Lead PCN Clinical Pharmacist and GP Clinical Supervisor.
Primary care experience would be an advantage, however, is not essential. Flexibility, enthusiasm and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor and other clinical pharmacists, enrolment onto a formal education pathway delivered through CPPE is also available (If not already complete)
Main duties of the job
- Review and interpret medication orders to ensure accuracy and appropriateness for individual patients.
- Monitor patient responses to medications and make necessary adjustments in collaboration with the healthcare team.
- Be a prescriber, or complete training to become a prescriber, and work with and alongside the general practice team.
- Provide drug information and education to healthcare professionals and patients regarding medication therapies.
- Participate in multidisciplinary rounds, offering expertise on pharmacotherapy to optimise patient outcomes.
- Conduct medication reconciliation following patient discharges.
- Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.)
- Ensure compliance with all relevant regulations and standards related to medication management.
- Maintain accurate records of medication use and interventions in accordance with hospital policies.
About us
We are a small but growing Practice, part of the wider South Lincolnshire Rural PCN. We work collaboratively as a team, which includes, GPs, Nurses, Paramedics, Social Prescribers, Specialist Neighbourhood Nurses, Physiotherapists, First Contact Physiotherapists, Mental Health Practitioners and Health and Wellbeing Coaches. We work closely with local agencies and providers of support services to provide a wrap-around service to our patients, offering a personalised care approach.
