JOB TITLE: Clinical Pharmacist

HOURS OF WORK: 37.5

LOCATION: Hybrid - Holbeach Medical Centre & working from home

REPORTING TO: GP Mentor in General Practice / Senior Clinical Pharmacist

RESPONSIBLE FOR: No Line management responsibility.

SALARY: £50,000 - £55,000

Primary Duties and Areas of Responsibility.

Long Term Conditions

See patients with single or multiple medical conditions, where medicines optimisation is Required. (e.g. COPD, asthma, ischaemic heart disease).

Review the on-going need for each medicine, ensure that monitoring is up to date, and support patients to get the most benefit from their medicines (i.e. medicines optimisation).

Make appropriate recommendations to Senior Pharmacists or GPs for improvements in treatment regimes.

Clinical medication reviews.

Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses and/or GPs on prescribing and monitoring.

Care home medication reviews

Undertake person-centred, clinical medication reviews with patients, and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.

Work with care home staff to improve the safety of medicines ordering and administration.

Domiciliaryclinical medication reviews.

Undertake clinical medication reviews with patients, and produce recommendations for the senior clinical pharmacists,nurses and GPs on prescribing and monitoring.

Attend and refer patients to multidisciplinary case conferences.

Management of common/minor/self-limiting ailments.

Managing caseload of patients with common/minor/self-limiting ailments while working within scope of practice and limits of competence.

Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.

Patient-facing medicines support.

Provide patient-facing clinics within your base practice, for patients with questions, queries and concerns about their medicines.

Telephone medicines support.

Provide a telephone helpline for patients with questions, queries and concerns about their medicines.

Medicines information to practice staff and patients.

Answer relevant medicine related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients. Suggest and recommend solutions.

Provide follow up for patients, to monitor the effect of any changes.

Unplanned hospital admissions.

Using prescribing support tools such as Eclipse, review the use of medicines most commonly associated with unplanned hospital admissions and readmissions, through audit and individual patient reviews, document findings in audit reports, conduct root cause analyses for any errors, and put systems in place to address any prescribing safety issues.

Management of medicines at discharge from hospital.

Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes, and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

Set up and manage systems to ensure continuity of medicines supply to high risk patients (e.g. those with medicine compliance aids or those in care homes).

To review medications for newly registered patients.

Signposting.

Ensure that patients are referred to other healthcare professionals for the required level of care within an appropriate timeframe e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.

Repeat prescribing

Produce, implement and follow a practice repeat prescribing policy helping to improve safety and efficiency.

Develop and implement ways to improve monitoring of high-risk medication such as anticoagulants, anticonvulsants, and DMARDs, etc

Monitor practice prescribing against local guidelines and formulary recommendations.

Develop prescribing guidelines for use within the Network.

Help increase uptake of Electronic Repeat Dispensing.

Risk stratification.

Identification of cohorts of patients at high risk of harm from medicines, through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.

Service Development.

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).

Information management.

Analyse, interpret and present medicines data to highlight issues and risks to support clinical decision making.

Medicines quality improvement

Undertake clinical audits, in areas of prescribing as directed by the GPs, feed the results back to the practice team, and implement changes in conjunction with the practice team.

Medicines safety.

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance, as well as any prescribing safety incidents identified within your base practice, ensuring any findings are shared with the PCN pharmacy team.

Implementation of local and national guidelines and formulary recommendations.

Monitor practice prescribing against the local health economy's RAG list, and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs).

Assist practices in seeing and maintaining a practice formulary that is hosted on the practices computer system.

Auditing practices compliance against NICE technology assessment guidance.

Provide newsletters or bulletins on important prescribing messages.

Education and Training.

Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Work collaboratively with the PCN team, develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

Suggest and recommend solutions using clinical knowledge and trusted pharmaceutical sources.

Attend relevant network and practice meetings, education and training, including regular PLT.

To analyse latest prescribing information and trends and subsequently present this in an appropriate format to clinicians within practices to promote good prescribing practice

Practice Targets.

Support practices to deliver on IIF, QIPP agenda, QOF and locally commissioned enhanced services.

To be aware of the prescribing budget and identify and action areas for cost improvement.

Work with PCN team to ensure the practice is compliant with CQC standards where medicines are involved.

Public Health.

Support public health campaigns.

Provide specialist knowledge on all public health programmes available to the general public.

Person Specification

Experience

Essential

• Have experience of or an awareness of common acute and long-term conditions that are likely to be seen in general practice.
• Must hold or be willing to work towards an independent prescribing qualification.
• Recognises priorities when problem solving, and identifies deviations from normal pattern; is able to refer to seniors or GPs when appropriate.
• Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
• Involves patients in decisions about prescribed medicines and supporting adherence as per nice guidelines.

Desirable

• Working In Primary Care.
• Working in a GP practice.

Qualifications

Essential

• Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
• Must hold or be willing to work towards an independent prescribing qualification.
• Must have completed/must enrol on the CPPE Primary Care Pharmacy Education pathway (PCPEP)
