Job summary

The South Lincolnshire Rural, Primary Care Network is offering an exciting opportunity for a Clinical Pharmacist to work within our Holbeach Park Road, GP practice.

Our Primary Care Network (PCN), we recognise the value that pharmacist's can bring to our practices and our patients and we look forward to growing our PCN pharmacy team. Our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients.

We are seeking an enthusiastic and forward-thinking pharmacist, to join our like-minded PCN. We want to find the best pharmacy talent to join our team. You will become part of a group of PCN Clinical Pharmacists working under the supervision of the Lead PCN Clinical Pharmacist and GP Clinical Supervisor.

Primary care experience would be an advantage, however, is not essential. Flexibility, enthusiasm and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor, other clinical pharmacists and enrolment onto a formal education pathway delivered through CPPE (if not already complete)

Main duties of the job

  • Review and interpret medication orders to ensure accuracy and appropriateness for individual patients.
  • Monitor patient responses to medications and make necessary adjustments in collaboration with the healthcare team.
  • Be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice team.
  • Provide drug information and education to healthcare professionals and patients regarding medication therapies.
  • Participate in multidisciplinary rounds, offering expertise on pharmacotherapy to optimise patient outcomes.
  • Conduct medication reconciliation during patient admissions, transfers, and discharges.
  • Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.)
  • Ensure compliance with all relevant regulations and standards related to medication management.
  • Maintain accurate records of medication use and interventions in accordance with hospital policies.

We are a small but growing Practice that is part of the wider South Lincolnshire Rural PCN. We work collaboratively as a team, which includes, GP's Social Prescriber, Specialist Neighbourhood Nurse and Pharmacy staff. We work closely with local agencies and providers of support services.

Primary Duties and Areas of Responsibility.

Patient facing Long Term Conditions

See patients with single or multiple medical conditions, where medicines optimisation is Required. (e.g. COPD, asthma,

ischaemic heart disease).

Review the on-going need for each medicine, ensure that

monitoring is up to date, and support patients to get the most

benefit from their medicines (i.e. medicines optimisation).

Make appropriate recommendations to Senior Pharmacists or

GPs for improvements in treatment regimes.

practice staff, other healthcare teams (e.g. community

pharmacy) and patients. Suggest and recommend solutions.

Provide follow up for patients, to monitor the effect of any

changes.

Unplanned hospital

admissions

Using prescribing support tools such as Eclipse and PINCER,

review the use of medicines most commonly associated with

unplanned hospital admissions and readmissions, through

audit and individual patient reviews, document findings in

audit reports, conduct root cause analyses for any errors, and

put systems in place to address any prescribing safety issues

Management of

medicines at discharge

from hospital

Reconcile medicines following discharge from hospitals,

intermediate care and into care homes, including identifying

and rectifying unexplained changes, and working with patients

and community pharmacists to ensure patients receive the

medicines they need post discharge.

Set up and manage systems to ensure continuity of medicines

supply to highrisk groups of patients (e.g. those with medicine

compliance aids or those in care homes).

To review medications for newly registered patients

Signposting

Ensure that patients are referred to other healthcare

professionals for the required level of care within an

appropriate timeframe e.g. pathology results, common/minor

ailments, acute conditions, long term condition reviews etc.

Repeat prescribing

Produce and implement a practice repeat prescribing policy helping to improve safety and efficiency

Develop and implement ways to improve monitoring of high-risk medication such as anticoagulants, anticonvulsants, and DMARDs, etc

Monitor practice prescribing against local guidelines and formulary recommendations

Develop prescribing guidelines for use within the Network

Help increase uptake of Electronic Repeat Dispensing

Risk stratification

Identification of cohorts of patients at high risk of harm from

medicines, through pre-prepared practice computer searches.

This might include risks that are patient related, medicine

related, or both.

Service Development

Contribute pharmaceutical advice for the development and

implementation of new services that have medicinal

components (e.g. advice on treatment pathways and patient

information leaflets).

Information management

Analyse, interpret and present medicines data to highlight

Medicines quality

Undertake clinical audits, in areas of prescribing as directed

by the GPs, feed the results back to the practice team, and

implement changes in conjunction with the practice team.

Implement changes to medicines that result from MHRA

alerts, product withdrawal and other local and national

guidance, as well as any prescribing safety incidents identified

within your base practice, ensuring any findings are shared

with the PCN pharmacy team.
