Job summary

An exciting opportunity has arisen for an Advanced Nurse Practitioner to join our friendly and supportive practice team. Chastleton is a long-established, successful training practice with a list size of 10500 patients, set just outside of Durham city. You will be joining us at an exciting time as we focus on adapting to the current changes within the NHS to provide the highest possible standard of holistic patient care.

Roles and responsibilities would include a mixture of acute care and chronic disease management, investigating and treating as appropriate.

The successful candidate will:

Be competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

Provide an opportunity for patients to receive timely care and negate unnecessarily delay in receiving treatment and without the need to refer to a GP.

Develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They will be autonomous in making decisions based on assessment, diagnosis and interpretation of test results.

Independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

Main duties of the job

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and wellbeing programmes

b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

c. Implement and evaluate individual specialised treatment plans for chronic disease patients

d. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

e. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

f. Request pathology services as necessary

g. Process and interpret pathology and other test results as required

h. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

i. Maintain accurate clinical records in conjunction with extant legislation

j. Ensure read/SNOMED CT codes are used effectively

k. Prioritise health issues and intervene appropriately

r. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual

s. Support the clinical team with all safeguarding matters in accordance with local and national policies

t. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

u. Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:

a. Participate in local initiatives to enhance service delivery and patient care

b. Support and participate in shared learning within the organisation

c. Develop an area of specialist interest, taking the lead within the organisation

d. Continually review clinical practices, responding to national policies and initiatives where appropriate

e. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

f. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

  • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy. Patient group directives, NICE and local clinical guidelines and local care pathways or make necessary referrals to other members of the primary care team
  • Manage undifferentiated, undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action

  • Have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) route; both pathways enable eligibility for an 'Advanced' digital badge issued by the Centre, demonstrating recognized educational and experiential preparation in advanced practice.
  • Qualified Independent Nurse Prescriber on the NMC register
