An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join our practice team.
Main duties of the job
- Act as the key contact for patients, supporting them to access appropriate services in a timely manner.
- Participate in regular MDT meetings and contribute to care planning and reviews.
- Coordinate care for patients on long-term condition registers (e.g., diabetes, asthma, hypertension), ensuring timely reviews
- Assist with recall processes and identify patients requiring appointments to meet QOF targets
- Tracking performance against QOF targets and supporting improvement initiatives
- Maintain up-to-date patient records using clinical systems on EMIS
- Complete administrative tasks assigned by the GPs
- Complete day to day scanning to support with document management
- Process new patient registrations and book them in with the nurse for an initial assessment
- Support with immunisation schedules for children
- Lead on QOF projects and quality improvement projects
- Liaise with multi-disciplinary teams for patient-centred care
- Support reception staff with incoming calls and patient queries
- Complete referrals requested by the GPs
- Book clinics for the nurse, mental health practitioner and clinical pharmacist
- Support with PCN targets by booking patients in and ensuring they have full reviews completed
