Job summary
Fareham and Portchester Primary Care Network - PCN is looking for a
motivated Paramedic, Advanced Nurse Practitioner - ANP, Advanced Clinical
Practitioner - ACP, or an experienced Nurse working towards a Level 7
qualification to join our dynamic and expanding same-day urgent care team and
home visiting team. This role is integral to our commitment to delivering
high-quality urgent care across four practices, focusing on acutely unwell
patients, as well as offering a unique opportunity to work a network of both
medical and non-medical professionals to deliver Coordinated, holistic care.
Main duties of the job
Key Responsibilities
Patient
CareProvide urgent care through telephone
triaging, in-practice consultations, and home visits.
Independent
and Collaborative WorkAbility to work autonomously during home visits and collaboratively within
a practice team.
Team
DevelopmentParticipate in and contribute to
training, mentoring, and peer support initiatives to promote team
development.
CollaborationLink with
doctors and other professionals during triage, home visits, and
multi-disciplinary team (MDT) meetings to deliver patient-centred care.
Holistic ApproachEngage
in proactive care by combining medical opinions, medication reviews,
mobility assessments, and health connections, with a focus on continuity
and follow-up.
Integrated Patient CareWork closely with a multidisciplinary team to coordinate and execute short
and long-term care plans, ensuring patients receive comprehensive support.
Follow-upEnsure ongoing
patient support by following up on referrals and care episodes, monitoring
progress, and adjusting care plans as necessary.

- Degree or equivalent in clinical work such as Advanced nursing or paramedic. Paramedics to have 3 year post qualification and 2 years consolidated learning
- Able to undertake clinical assessment of a patient having completed relevant course.
- Experience in Minor illness triage.
- Ability to work as an integrated team member
- Excellent communication skills
