Fareham & Portchester Primary Care Network are delighted to announce that we have an exciting opportunity to join our team in the role of Urgent same day care co-ordinator based at The Portchester Practice
We are looking to recruit an individual with experience of working in primary care who will support our appointment access transformation. We run an appointment system consisting of GP-led same-day telephone triage for urgent matters and eConsult online consultation for less urgent access. The successful candidate will be expected to work alongside a GP to learn and develop care navigation strategies to facilitate the most suitable care disposition for patients seeking our services. It is expected that this role will develop to be able to work autonomously making care navigation decisions and to build a team of fellow care navigators and data gatherers. Linking in with the wider Social Prescribing team and Health and wellbeing coach
Successful candidates will be expected to enrol in, or already undertaking or qualified from appropriate training as set out by the Personalised Care Institute
Main duties of the job
Review and navigation of Urgent same-day care triage matters and online eConsults
Work with the GP team to lead the effective triage of incoming online consultations
Liaise with the patient to take a more detailed history or gather additional information when required.
Liaise with the supervising GP to discuss the most appropriate way of managing an online consultation.
Manage patient needs by answering queries, and booking appointments with practice and wider PCN teams, including Clinical Pharmacists, Social Prescribers, Home Visiting, and Paramedic teams, and First Contact Physios.
Assist with Scheduling patient treatments or other procedures
Ensuring accurate and legible notes of all consultations and treatments are recorded in patients notes
Data entry and coding of patient information pertaining to practice QOF requirements
Support patients to take up training and employment and to access appropriate benefits where eligible via referral to the Social Prescribers and health and wellbeing coach
Provide appropriate coordination and navigation for people and their carers across health and care services.
Attending and participating in PCN and practice meetings as required
Work in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider and Health and Wellbeing Coach(es). Or other PCN members for appropriate navigation of care
About us
We are an expanding 10,150 patient practice in Portchester, situated between Fareham and Portchester on the south coast. The practice is well-regarded locally and is ranked in the top 6% of GP practices in England in the 2021 survey. We have an excellent primary care team who are committed to providing high quality patient care. The practice has a strong training ethos and comprises 5 GP partners, 1 salaried GP, 2 retained GPs and several GP registrars. The nursing team consists of 1 lead nurse, 2 practice nurses, 2 healthcare support workers and a phlebotomist. We have an urgent care nurse workingalongside the duty GP for same day access Monday to Wednesday and plan to involve our PCN paramedics for this role on Thursday/Friday in the near future. We have a strong management and administrative team to support all our clinicians. We value teamwork, mutual support and a friendly working environment.
We part of a wider Primary Care network that comprises Paramedics, Nurses, General Practice Assistants, Care-coordinators, MSK, Clinical Pharmacy team, and Home Visiting Service.
