Due to our growing list size, Violet Lane Medical
Practice is seeking to recruit a qualified advanced
nurse practitioner (prescriber) to join our motivated, friendly and supportive
GP Practice. The successful candidate
will be confident in independently managing acute cases of minor illnesses
arising from the clinical triage system.
Seeing and reviewing patients with long term conditions. Willing to take part as a senior nursing
colleague in leading, motivating, and training staff and students. Supporting the triage and care navigation of
patients as well providing care to patients face to face, over the telephone
and sometimes in their own homes.
Working hours: Full time but 24 hours plus will be
considered.
Salary: Highly competitive.
Main duties of the job
The Advanced Nurse Practitioner
Prescriber (ANP) will act autonomously within their professional scope of
practice providing care for patients face to face and via telephone
consultations, ensuring clinical safety and excellence. They will need to prioritise and triage the
needs of patients appropriately, making any necessary referrals for
investigations in the appropriate manner.
They will work collaboratively with the
general practice team and multi-disciplinary external agencies where
appropriate to meet the needs of patients, supporting the delivery of policy
and procedures and contract delivery. To work at this
level, NMC requirements for advanced practice must be met.

heartland of its community in South Croydon, Violet Lane Medical Practice has
been a mainstay with a stable patient list size of 11,000. With the addition of
two motivated GP partners earlier this year alongside our experienced partner
of 25 years, this forward-thinking practice is looking to develop and innovate
to ensure high quality patient care is maintained and health inequalities are
tackled. Having recently registered as a training practice we now consist of 3
partners and 4 salaried GPs in addition to 2 nurses, GP trainees, medical
students and ARRS staff. Staff are hard-working whilst also maintaining a

Advanced Nurse Pracitioner - Prescriber - Job Description
Job Summary
The Advanced Nurse Practitioner Prescriber (ANP) will act autonomously within their professional scope of practice providing care for patients face to face and via telephone consultations, ensuring clinical safety and excellence. - Help develop and set up new patient services and participate in initiatives to improve existing patient services.
- Be aware of and contribute to planning and delivery of commissioning within the Primary Care Network.
- Promote a learning environment for patients, students, nurses and other health professionals.
- Contribute to the planning and implementation of the teaching and training for existing staff, such as medical students, nurses, GP registrars, health care assistants, and ARRS roles within the Practice.
- Encourage and develop teamwork within the Practice.
- Help the Practice operate in a cost-effective manner.
- Participate in audits and inspections as appropriate.
Clinical Nursing Practice
- Assess, diagnose, and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency including making use of skills in history taking, physical examination, problem solving and clinical decision making.
- Diagnose, manage, and review acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.
- Work as part of a multi-disciplinary team to deliver safe and high-quality care.
- Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
- Delivery of care in line with NSF, NICE guidelines and evidence-based care.
- Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
- Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
- Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
- Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
Care Management
- Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.
- Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals).
- Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking, IIF, PCN DES etc.
- To monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
Health Improvement
- Management of health screening programmes and ensures other team members are aware of health priorities and screening objectives and programmes.
- Innovation in practice to meet ongoing demands on the practices delivery of care.
- Improve health outcomes for patients by introducing new developments and can demonstrate them as reports and presentations where appropriate.
- Supports practitioners in reporting the practice population to promote health improvement and disease prevention.
- Makes referrals to appropriate agencies (statutory and voluntary) where necessary.
