Job summary

We are looking for a suitably qualified professional with high clinical standards to undertake on the day appointments/same day triage for patients registered across the PCN network.

You will need to be able to work autonomously as well as part of a multidisciplinary team and be able to demonstrate safe, critical thinking in clinical decision making and offer expert care to our patients.

The role will be based at Ragsdale House, Woodville but also may be required to work from our member Practices should the need arise. All of our Practices are located in the small geographic area of Swadlincote.

Main duties of the job

1. Be able to work autonomously and be accountable for your own professional actions.

2. Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.

3. Triage and assess undifferentiated patients with a range of acute and chronic medical conditions. Take histories, make appropriate physical examinations, formulate differential diagnoses, and carry through management treatment plans ensuring relevant follow up. Identify red flags or serious underlying pathology and take appropriate action. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

About us

We are an established forward thinking PCN with a team of five GP practices within the small demographic area of Swadlincote, holding a list size of approximately 58,000. Our PCN Team currently consists of a Paramedic, 5 Trainee Nurse Associates, 2 Social Prescribers, 5 Health and Wellbeing Coaches, 3 Mental Health Practitioners, 5 Clinical Pharmacists and 6 Care Co-Ordinators along with a Leadership Team consisting of the Clinical Director, Deputy Clinical Director, PCN Manager and Deputy PCN Manager.

We also have a well-established home visiting service team that work from the PCN site which includes GPs, Advanced Clinical Practitioners and Enhanced Care Practitioners.

Job description

Job responsibilities

3. Triage and assess undifferentiated patients with a range of acute and chronic medical conditions. 4. To prioritise health problems and intervene appropriately, including initiation of effective emergency care.

5. Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.

6. Discuss with registered GP and, where appropriate, make referrals to other health professionals in primary and secondary care.

7. Prescribe medication as appropriate as an Independent Prescriber.

8. Maintain accurate record keeping in SystmOne.

9. Request and interpret tests appropriate to patients need.

10. Provide effective clinical leadership, development and mentoring to operational personnel on behalf of the PCN, promoting and ensuring high quality out-of-hospital care is delivered by competent employees.

11. Assist in the development of an integrated approach to the delivery of care involving all relevant stakeholders including health and social care organisations within the primary care field.

12. Participate in work towards PCN DES, Practice QOF and Enhanced Service targets and specifications where applicable to support the 5 member practices.

13. Deliver evidence based clinical practice by utilising applied knowledge of clinical audit, critical appraisal of research, research methodology, ethical review and research governance.

14. Make appropriate use of local and national guidance documents.

15. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance to evidence-based practice and national and Practice protocols, and within scope of practice.

16. Work as an effective member of a multi-professional team.

17. Maintain an up-to-date knowledge of clinical practices, ensuring that these are reflected in compliance with the PCNs policies and procedures.

18. Demonstrates ability in all four pillars of Advanced Practice, clinical practice, leadership and management, education and research.

19. Assist the GPs, Managers and Partners by promoting and establishing a healthy and safe working environment for all personnel. Provide clinical advice and feedback on matters of health and safety, risk assessments, accident reporting and investigations.
