We are looking for a highly skilled Operational and Transformation Lead to deliver effective leadership, pro-active supportive management and development of primary care services within Venn Primary Care Network
This is a busy but extremely rewarding and vital role to aid the development of our PCN. In this role you will have the opportunity to use your creativity to develop effective strategies, demonstrate fully inclusive engagement and provide specialist workforce expertise to support the development and implementation of the future objectives of Venn Primary Care Network.

Main duties of the job
- Day to day operational delivery of key projects and priorities to enable VENN to achieve its strategic and contractual objectives.
- Provide expert support with regards to securing new business opportunities for the PCN and member practices.
- Work with the Strategic Lead and Clinical Director to deliver strong, clear and safe operational frameworks and standards for the services within the PCN.
- To work with the BI lead ensuring robust service and contractual reporting in partnership with the Strategic Lead and Clinical Director
- Act in the capacity of key operational manager with partners and commissioners as appropriate.
- To work with the Strategic Lead and Clinical Director andfrontline PCN teams and service users to ensure we provide high quality, accessible care.
- To adopt a restorative approach to leadership embedding the principles of open and positive communication for all members of the team and external partners.

GENERAL
1. The above duties may involve having access to information of a confidential nature, which may be covered by the GDPR. Confidentiality must be maintained at all times.
2. The post holder must be flexible to ensure the operational needs of the PCN are met. This includes the undertaking of duties of a similar nature and responsibility as and when required, throughout the various work places in the company.
3. To promote and implement all Employment Legislative requirements.
4. The Health and Safety at Work Act (1974) and other associated legislation places responsibilities for Health and Safety on all employees. DIMENSIONS:
Responsibility for Staff:
- Day to day advisory responsibility for the clinical governance and risk management of services within the PCN GP services. To support the board members to deliver high quality care and to work with the Strategic Lead and Clinical Director to implement new services and innovations as and when required
- Organisation and prioritisation of workload and delegation of work as delegated by the board.
- Resolving staffing and operational issues on a day to day basis
- To ensure participation in supervision, have continuing development opportunities, participate in their own Development Review Objectives
- To work with and support the Clinical staff and lead clinicians within VENN PCN to deliver high quality care
- Work with the Strategic Lead and Clinical Director to have in place a structure that enables decision making and managing resources on behalf of the PCN on the ground as well as one that supports innovation, understands risks and helps people to think through the wider implications of changes in practice.
- To undertake projects/reports as identified by the Strategic Lead and Clinical Director .
- Understands professional accountability and appropriate Code of Conduct. This will include the review of standards of care, examination of critical incidents and identification of expert and sub-standard care in a way that promotes a no-blame culture whilst meeting the requirements of the PCN and monitoring SIs and incidents.
- Responsible for recording serious and untoward incidents, ensuring the appropriate documentation is completed, taking any follow up action as required.
To develop and monitor the PCNs training needs plan.
Responsible for the day to day management of the PCN administrative, clerical and support functions within the area of responsibility. Working with and supporting frontline practitioners to ensure pro-active employment opportunities and services with the right number of staff with the appropriate skills, knowledge and attitude to deliver on the integrated clinics and services provided by the PCN. This will include supporting the recruitment and retention of staff, team performance, sickness absence, capabilities, discipline and grievance procedures, training and development, workforce planning and job/role development of joint PCN staff.
- Within the area of responsibility provide professional leadership to frontline staff.
- To work proactively in the PCN to develop and deliver services as agreed per national guidance.
Responsibility for Customers/ Clients/ Consumers:
- Close working with acute trust providers, Local Authority and ICBs. and community and mental health services.
- Ensuring that the skills of the teams reflect priority needs and that a culture is established that facilitates creative thinking and uses the skills experiences of staff to allow integrated working across traditional and hierarchical boundaries.
- Ensuring services are accessible and inclusive to all groups within the PCN population such as black and ethnic minorities, excluded groups and those with additional need.
- Work with others to establish and strengthen current and future partnerships for service delivery. Ensuring that clinical links are made into the local area structures and that system are established that can maximise the engagement of communities, service users and the frontline staff in the delivery of modernised services on behalf of the PCN.
- Work with other key personnel to secure funding for new services, which are in line with commissioning priorities for improving health and reducing inequalities.
Responsibility for Strategy, Business Plans and Budgets:
- Monetary To support the finance lead as requested on behalf of the PCN.
- Adherence to and developing policies such as Standing Financial Instructions, Standing Orders and Anti-Corruption policies.
- Responsible for the effective utilisation of property and equipment within VENN PCN
- To complete annual risk assessment in line with Health and Safety legislation and Clinical Governance measures.
- To liaise with Finance Lead to ensure expenses, meeting attendance payments and other PCN funded work is collated.
- Responsible for ensuring accurate reporting of performance measures and timely submissions
- Making sure that measures of activity are in place that provide meaningful information, health outcomes and reflect professional standards
- To oversee the PCN administrator to enable them to establish and maintain efficient office systems.
