Job summary

We are seeking to appoint a part-time (approximately 18 hrs per week) experienced Diabetic Nurse Specialist to join our friendly team.

Main duties of the job

You will be providing one to one care of our Diabetic Patients alongside our dedicated GP Lead, Practice Nurse and HCA.

About us

We are a dedicated team of 6 Partners, 2 Salaried GP, 2 ANPs, 4 Practice Nurses, 2 HCA/Phlebotomists offering an excellent quality of care to our Practice population consisting of a list size of approximately 12,400 patients. Our Main Surgery is situated in Broughton Astley with a Branch Surgery in Ullesthorpe. The Practice is part of the Hinckley and Bosworth Medical Alliance (Federation) and the Fosseway PCN.

Job responsibilities

Skills desired/required:

  • Evidence of training in Diabetes Management
  • Completion of the CaReMe course or working towards completion
  • Nurse prescriber qualification or plans to work towards achieving.
  • Experience of insulin initiation and management.
  • Home visits when required.

We are/have:

  • Teaching practice with a GP registrar
  • Support from clinical Pharmacists, First-Contact Physiotherapist, Mental Health Practitioner, Social Prescribers, and Nurse Associates.
  • GPwSI in Cardiology, Dermatology and Womens health.
  • Nurses specialised in Diabetes, Asthma/ COPD, care planning and nursing home care.
  • Dispensing Practice
  • SystmOne Clinical System
  • High achieving Practice in all areas of QOF/QIPP, Enhanced Services (including implants, coils, joint injections etc), IIF/Basket of Services, Dispensing Quality Services.
  • CQC inspection rating of good in all areas
  • Excellent patient satisfaction results
  • Regular staff events
