Job summary
Summary
The post holder will be responsible for managing the Prescription Team and will work alongside the Lead GP for Prescribing and Clinical Pharmacist in supporting the team to deliver safe, cost-effective prescribing to the practice population. The post holder will process administrative medication changes from clinic letters (to be checked by a clinician) and manage the repeat medication ordering process. They will work closely with the Reception Manager and other Team Leads to provide a fair, safe and effective service for patients.
Main duties of the job
Practice Development and Clinical Governance
Patient & Community Services
Support patient education by developing and improving patient information.
Manage repeat medication ordering process.
Maintain and manage prescription policies.
Provide a first point of contact for patients with prescribing related queries.
Develop excellent working relationships between the Practice and their local community Pharmacies and care/nursing homes.
Collaborate with pharmacists, doctors, and other healthcare professionals to ensure proper medication therapy. Bringing medication inline.
Communicate effectively with healthcare providers to clarify prescriptions or adjust medications based on patient needs.
Having regular contact with care homes aligned to the surgery, dealing with queries that arise.
Managing out of stock medication alternatives
Reduce waste by reducing the inappropriate issue of prescriptions
Promote cost effective prescribing
Work alongside Teams from other departments to provide the best service for patients.
Attend, participate in, and contribute to staff meetings.
Maintain appropriate records.
