What do we need?
We are looking for an engaged, positive, friendly, hard-working and resilient individual to join our clinical team at Alvanley Family Practice.
We're working in an area with a very busy day-to-day operation, so we need someone flexible and resilient.
The post would be either full or part time (please state your preferred hours on the application). Currently we allocate one on-call clinician per day, so you would be expected to be part of this rota.
In our PCN, we have access to a Pharmacy Hub, First Contact Physio, Social Prescriber, Care Coordinator and Health Coaches, as well as Workflow Optimisers to help reduce GP workload.

Main duties of the job
A Typical Day
- A mixture of minor illness and other appointments based on clinical need.
- Possibly other responsibilities such as support with the care home.
- Completion of clinical administration.
- Some flexibility can be offered around start times around childcare etc. - please discuss with us.
- We are a very busy practice, but we try our best to avoid extras where possible.
About us
Alvanley Family
Practice is part of Woodley Health Centre, in a desirable area close to the
M60. We have very high rates of QOF achievement, and excellent patient survey
results.
We feel our
relationship with our patients makes us unique. We have a long-running and well
established set of Patient Champions, and have a range of community schemes
alongside our patients including a walking group and an allotment. We find
these schemes have helped solidify our place in the community, and encourage a
wider range of patients to engage with our services.
We have just over
6,000 patients on our list, so we are a busy practice. Our partners and
management team are highly focused on using technology to allow colleagues
across the building to work smarter, and this is an ongoing journey for us.
We are a training
practice so have experience in helping to guide newer members of the profession
through the early stages of their careers.

Job responsibilities
The Advanced Practitioner works autonomously and is accountable for own professional actions.
Undertakes Clinical Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people
accessing primary health care services.
Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic
medical conditions.
Assesses diagnoses and treats patients in surgery or own home who require acute medical attention and refer to
appropriate agency.
Refers patients to other medical specialities for assessment if deemed appropriate.
Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and
voluntary organisations including referral for x-ray.
Undertakes and interprets a range of diagnostic tests and routine clinical procedures e.g. cytology, venepuncture, blood
pressure monitoring etc.
Makes critical judgements of the highest order to satisfy the expectations and demands of the job e.g. first contact with
patients presenting acutely and manages care in the presence and absence of protocols.
Makes decisions where precedents do not exist, where appropriate without recourse to others e.g. direct referral of patients
to Hospital Consultants without prior reference to other medical colleagues.
Advises and supports others where standard protocols do not apply.
Works within professional guidelines and codes of conduct.
Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
Extended and supplementary prescriber.
