Job summary

We are looking for a Clinical Pharmacist with

Undergraduate degree in Pharmacy (MPharm or equivalent)

Registered with the General Pharmaceutical Council

Minimum of 3 years post qualification experience

Ideally holds an independent prescribing qualification

Previous General Practice experience is desirable

This is an exciting opportunity for an enthusiastic and forward-thinking local Pharmacist to join our established and growing team in providing meaningful, patient centred support in general practice.

Main duties of the job

Work as part of a multi-disciplinary team, in a patient facing role

Work in the Long Term Conditions clinics

Undertake clinical medication reviews

Support general practice staff with prescription requests, and medication queries

Undertake safe prescribing practice

Work with the clinical staff with project work and audits

Successful candidates can expect:

PMS practice with 17 000 patients

7 Clinical Partners, and a Managing Partner

4 Salaried Doctors, 2 part time Pharmacists, 1 Pharmacy technician, 3 Practice Nurses, 2 Health Care Assistants, Counsellor, and a Social Prescriber.

Training Practice (STs, F2s and medical students)

EMIS web, High QOF and LCS achievements

Weekly clinical and practice meetings and a daily morning Coffee break

Additional services (family therapy, minor surgery, anticoagulation, LARC fitting)

Involvement in PCN, ICB , LMC, Federation and a collaborative provider of Urgent Care at the local Trust

Working within the practice-based team to undertake medication reviews particularly in high risk groups such as: Frail elderly, patients with long term conditions etc

Polypharmacy

Renal impairment

Hepatic impairment

Substance misuse

Patients on high-risk medicines

STOPP / START identified patients

Revolving door Hospital admissions

Improve prescribing practice through educational support for all prescribers within the practice

Lead on where changes in evidence require changes in prescribing across patient population where a drug is withdrawn, or indications change.

Liaise with colleagues in community pharmacy to align support for medicines adherence such as MUR and NMS.

Support improvements in clinical care through practice-based audit and implementing change.

Prescribing advice to prescribers in practice temporary non-availability of drugs.

Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines

Desirable

  • Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience independent prescriber or working towards/intent of gaining independent prescribing qualification
