at a masters level aware or equivalent that encompasses the four pillars of
clinical practice;Independent prescribing qualification
Hours:Approx.
37 hrs per week as per clinicalteam
rota (part time and job shareconsidered)
Our Advanced Nurse
Practitioners are highly appreciated within the surgery, working closely with
the GPs and the practice teams of all five of our practices: Dr Koh and Trory,
Gladstone House Surgery, Hart Medical Practice, Seaton Surgery, and West View Millennium
Surgery.
We have a team of
clinicians and non-clinical staff who all assist with running the surgeries on
a day to day basis and our surgeries provide care for all patient
demographics. Across Hartlepool Network, we currently have 32,000
patients, but our practice list sizes are increasing all the time. The GP
practices within Hartlepool Network pride themselves on delivering a high
standard of clinical care and being a supportive network to the staff who work
within it.
Job Purpose
ANPs are a vital resource in
general surgery, assisting our GPs with urgent care for patients as well as
also managing ongoing patient care of areas to which they are competent. By
interacting more with our patient population we hope to teach patients about
the different routes into primary care and find the way that suits them best.
Reporting to the Lead GP and Lead
Nurse the ANP will:
Work
within all relevant policies and procedural guidelines
Work
within professional guidelines and codes of conduct
Adhere to
the requirement that information relating to patients, colleagues, other
healthcare workers or the business of the company may only be divulged to
authorised persons in accordance with the company policies and procedures
relating to confidentiality
Key Responsibilities
Clinical
Working independently
and alongside GPs to identify appropriate care paths and treatment methods for
patients through face to face appointments, telephone consultations and home
visits
Making
professional, autonomous decisions in relation to presenting problems whether
self-referred or referred from other health care professionals
Using
clinical nursing practice at an advanced level to deliver quality care and
advice to patients and demonstrate an in depth knowledge of the options for
achieving control of symptoms and assessing the health needs of patients with
undifferentiated and undiagnosed problems
Perform
safe triage of patients when necessary via telephone, video, econsultation and
face to face consultations
Prescribe
medication appropriate to patient needs or refer the patient to a community
pharmacy who will provide designated minor aliment services and prescription
advice
Provide information
and advice on prescribed or over-the-counter medication on medication regimens,
side-effects and interactions
Support
and educate patients to adopt and promote healthy lifestyles and apply
principles of self-care
Maintain
a concept of nursing which incorporates physiological, social and spiritual
components
Supporting
surgery staff performance through monitoring and giving feedback
Management
of Chronic Disease and specified QOF areas
COPD
Management and Results Interpretation

Therapeutic

In
general the post holder will be expected to undertake all of the normal duties
and responsibilities associated with an Advanced Nurse Practitioner working
within Primary Care
Communication
Ensuring
that all written documentation is completed and that appropriate data is
collected.
Promoting
and maintaining effective liaison, communication and relationships with all of
the providers involved in caring for the patient
Utilise
and demonstrate sensitive communication styles to ensure patients fully understand
advice and instructions.
Recognise
peoples needs for alternative methods of communication and respond accordingly
Promote
and practice excellent verbal communication skills with patients and colleagues
Maintain
effective communication within the project team and with external stakeholders,
providers and organisations as appropriate
Communicate
with and support patients with complex mental health issues
Quality
Recognise
and work within own competence and professional code of conduct
Produce accurate,
contemporaneous and complete records of patient consultations to agreed
standards
Effectively
manage own time, workload and resources
Support
and participate in shared learning across the company and wider organisation
In
partnership with other clinical teams, collaborate on improving the quality of
health care responding to local initiatives as appropriate
Evaluate
patients response to the consultation and the effectiveness of care
Understand
and apply legal issues that support the identification of vulnerable and abused
children and adults and be aware of the statutory child/vulnerable patients
health procedures and local guidance
Health and Safety
Using
personal security systems within the workplace according to practice guidelines
Identifying
the risks involved in work activities and undertaking such activities in a way
that manages those risks
Awareness
of local and national standards of infection control and cleanliness and regulatory/
contractual / profession requirements and good practice guidelines
Active
observation of current working practices across the practice in relation to
infection control, cleanliness and related activities, ensuring that procedures
are followed and weaknesses / training needs are identified and escalating
issues as appropriate
Maintenance
of sterile environments and awareness of spillage control procedures
Correct
and safe management of the specimens process including collection, labelling,
handling, use of correct and clean containers, storage and transport
arrangements
Waste
Management including collection, handling, segregation, container management,
storage and collection
