Job summary

ANPs are a vital resource in general surgery, assisting our GPs with urgent care for patients as well as also managing ongoing patient care of areas to which they are competent. By interacting more with our patient population we hope to teach patients about the different routes into primary care and find the way that suits them best.

Main duties of the job

Working independently and alongside GPs to identify appropriate care paths and treatment methods for patients through face to face appointments, telephone consultations and home visits

Making professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other health care professionals

Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems

Perform safe triage of patients when necessary via telephone, video, econsultation and face to face consultations

Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor aliment services and prescription advice

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Support and educate patients to adopt and promote healthy lifestyles and apply principles of self-care

Supporting surgery staff performance through monitoring and giving feedback

Management of Chronic Disease and specified QOF areas along with 111 Triage, administrative and medication tasks, the post holder will be expected to undertake all of the normal duties and responsibilities associated with an Advanced Nurse Practitioner working within Primary Care

Reporting to the Lead GP and Lead Nurse the ANP will:

Work within all relevant policies and procedural guidelines

Work within professional guidelines and codes of conduct

Adhere to the requirement that information relating to patients, colleagues, other healthcare workers or the business of the company may only be divulged to authorised persons in accordance with the company policies and procedures relating to confidentiality

Key Responsibilities

Clinical
