evidence-based care across chronic disease management, health promotion,
screening, and preventative services.

Main duties of the job
Providing clinical care in the treatment room, including wound care, injections, and immunisations.
Supporting the management of long-term conditions such as diabetes, asthma, and hypertension.
Performing routine health checks, screenings, and vaccinations.
Working collaboratively with the Lead Practice Nurse, GPs, and the wider multidisciplinary team.
Contributing to the development and improvement of primary care services in response to increasing demand.
We are looking for individuals with proven Primary Care nursing skills and experience, who can demonstrate safe, evidence-based clinical practice, excellent communication, and a commitment to patient-centred care.
Conduct routine health checks, screening, immunisations, and travel vaccinations, as well as treatment room procedures including wound care and minor procedures. Support patient self-management through education, advice, and health promotion.
Work collaboratively with GPs, the Lead Practice Nurse, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.
Contribute to service development and quality improvement initiatives, helping to shape and improve the patient experience at the practice.

- ECGs and spirometry
- Undertake new patient health checks and routine screening
Chronic Disease Management
Support the management of patients with long-term conditions, including:

- Asthma and COPD
- Hypertension
- Carry out reviews, monitoring, and patient education
- Support patients to self-manage their conditions

- Administer medications in accordance with PGDs and scope of practice
- Provide advice to patients on medication use, side effects, and compliance
Patient Management & Administration
- Request and process pathology tests as appropriate
- Maintain accurate, clear, and contemporaneous clinical records
- Refer patients to GPs or other healthcare professionals when appropriate
Team Working
- Work collaboratively with GPs, nursing colleagues, and the wider MDT
