Job summary

We are looking for a Pharmacy Technician to join our team who are committed to delivering the highest quality clinical outcomes in a supported environment, where you will be valued for your skills and expertise. You will be responsible for the delivery of the highest standards of care & services. You will be required to work from Monday to Friday and the possibility of some Saturdays. Enthusiasm and flexibility are essential.

Main duties of the job

Your role will be to support our GPs and Clinical Pharmacists taking a central role in clinical aspects of shared care protocols, liaising with hospitals, pharmacies and patients

Job description

Job responsibilities

a. To support the pharmacist with medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly ;

b. undertaking patient facing and patient supporting roles to ensure effective medicines use, through shared-decision making conversations with patients;

c. supporting the practice based pharmacist to provide medication reviews and Structure Medication reviews;

d. supporting, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists.

e. taking a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients;

f. To provide advice to GPs, staff and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness

g. supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing;

h. assisting in the delivery of medicines optimisation and management incentive schemes and patient safety audits;

i. To assist in the appropriate monitoring and management of prescribing the budgets and to liaise with the medicines management team when required;

j. To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing;

k. supporting the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g. use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services;

Qualifications

Essential

  • BTEC/NVQ Level 3 or equivalent in Pharmaceutical Sciences
  • Registered with the General Pharmaceutical Council
  • Demonstrable commitment to professional development

Experience

Essential

  • Experience of Microsoft Office software
  • Advanced numeracy skills to GCSE Level C or above
  • Excellent keyboard and computer skills including use of Microsoft Office
  • Demonstrable excellent communication skills
  • An understanding, acceptance and adherence to the need for strict confidentiality
  • Ability to use own judgement, resourcefulness and common sense
  • Willingness to be trained and developed
  • Experience of dealing with the public/patients
  • Ability to work at part of an integrated multi-skilled team
  • Take responsibility for, and accept outcomes of, your own decisions
  • Able to work under pressure
  • Able to work in a changing environment
