Job summary

Swaffham & Downham Primary Care Network (PCN) have an exciting opportunity for an enthusiastic, dynamic and innovative PCN Project Manager to support in the PCN development and improvement. This role is fundamental to innovative and integrated services across seven practices based in Swaffham and Downham Market in West Norfolk.

Main duties of the job

The role will oversee the implementation and operational delivery of Enhanced Access and additional Patient Access Services. These services form part of the agreed contract the PCN is signed up to deliver.

To manage services and projects accordingly, clarifying objectives and priorities for network delivery in line with operational capacity.

To provide line management to PCN employed staff, to ensure that they are embedded in the PCN.

The PCN Project Manager will form an integral part of the PCN leadership team working alongside the PCN Manager, Clinical Director, Executive Committee members, Practice Managers, partner organisations and local communities, to ensure the delivery of the Primary Care Network priorities.

Based in West Norfolk, our PCN serves over 55,000 patients across our seven member Practices of; Manor Farm Medical Practice, Plowright Surgery, Campingland Surgery, Litcham Health Centre, Howdale Surgery, the Hollies Surgery and Bridge Street Surgery.

Job description

Job responsibilities

Responsible for the day to day management of the Enhanced Access provision to deliver the agreed outcomes within time, cost and quality constraints.

Support PCN Manager in implementation and management of Improved Patient Access, PCN Capacity measures, and digital transformation.

Develop new policies, standards, SOPs and guidelines as required.

Discuss with other members of the team how the policies, standards and guidelines will affect work.

Monitor and review effectiveness of new services to review impact and value for money.

Focusing delivery to reflect local priorities, health inequalities or population health management.

To actively recruit clinical staff to support enhanced access provision across the PCN.

Responsible for monitoring demand and capacity and ensuring any clinician referral and patient access issues are identified and resolved promptly.

Working with people and their carers and Primary Care Staff to organise and prepare for Annual Health Checks, enabling the patients to be actively involved in managing their care and supported to make choices that are right for them.

Support the PCN and its member practices in implementing project and delivery plans in relation to meeting the DES specification, health inequality and social prescribing remits.

You will line manage, develop, and support staff, while also leading on improving the efficiency and quality of clinical and operational processes and the smooth running of the service and always working closely with clinical teams.

Minimise organisational and PCN risk and be confident to challenge decisions if not in line with best practice.

Work closely with the PCN Manager to identify and develop the functionality in key projects including workforce, ICT and digital transformation, operational and finance workstreams.

Provide regular reports and updates to the PCN Board.

Be aware of performance and quality indicators for all aspects of Primary Care.

To consider the bigger picture, identifying linkages and dependencies between different areas of the overall DES, and supporting joined up working across the PCN.

Support in investigating PCN related complaints and assist in investigations.

Liaise and work with different practices on an ad-hoc project basis.

Support the recruitment processes for new PCN staff, from interview to induction plans, investigations.

Conduct appraisals and performance management of PCN staff

Provide performance data for a range of PCN activities as required

Use Excel spreadsheets to display and input performance data

PUBLIC HEALTH

Work with local stakeholders and providers to participate in Public Health campaigns as appropriate

Actively work toward developing and maintaining effective working relationships both within and outside the practice, PCN and locality

Foster and maintain strong links with all services across locality
