Chester Central Primary Care Network

This job is now closed

Job summary

Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system

Following the release of ARRS GP funding we are looking for newly qualified (within 2 years) to join Chester Central Primary Care Network on a fixed term salaried contact until 31 March 2026. We will consider applications for a minimum of four sessions with a start date as soon as possible.

There won't be a need to complete home visits or be included in the practices administrative duties, which will allow the newly qualified GP to settle into the GP role without the additional pressure of administrative duties.

Our clinical system for all practices is EMIS Web and we have a structured administrative team to help with letters, results, medication management, etc.

Working as part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group

We reserve the right to close the application early, so would encourage early application

Main duties of the job

  • Working across three of the six practices you will undertake a variety of duties including face to face and telephone consultations and queries, with no home visits or paperwork.
  • Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation;
  • Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems;
  • Screening patients for disease risk factors and early signs of illness;
  • Developing care plans for health in consultation with patients and in line with practice disease management protocols;
  • Recording clear consultation notes to agreed standards;
  • Collecting data for audit purposes and complying with QOF requirements;
  • Compiling and issuing computer-generated acute and repeat prescriptions;
  • All general duties and responsibilities associated with a GP working within primary care.
