Job summary

  • To act as an autonomous practitioner, utilising knowledge and skills to provide care, treatment and management of acute and chronic health care problems to the whole practice population.
  • Provide evidence-based care using appropriate national and local guidelines.
  • Act in collaboration with colleagues from within and outside the practice as a member of the multidisciplinary team.
  • Engage in and actively encourage training and development of self and other staff.

  • Exercise a high degree of professional autonomy and critical judgement. Make professionally autonomous decisions utilising knowledge and skills gained via study at Degree and post graduate levels and lengthy experience in a variety of settings
    • Proactively assess patients with previously undiagnosed and undifferentiated health care problems.
    • Assess health care needs, screen patients for risk factors and disease and provide health promotion advice.

    Main duties of the job

    • Treat patients utilising highly developed knowledge and skills, or refer to a range of primary and secondary care services as appropriate.
    • Order and interpret tests and investigations required for diagnosis and management of patients.
    • Review and monitor care plans and therapeutic interventions appropriate to patient need and in accordance with evidence-based practice and national and local guidelines.
    • Prescribe in an independent and supplementary role
    • Engage with other members of the practice and multidisciplinary team to develop effective, evidence-based care for the practice population.
    • Contribute to the development of joint standards of care, clinical guidelines and management pathways for utilisation within practice
    • Maintain a high standard of professional care at all times in accordance with the NMC Code
    • Work in collaboration with the PCN practices, Leadership team and nursing colleagues
    • Maintain own professional and clinical expertise via participation in clinical activities, professional updating and clinical governance activities.
    • Act as a positive role model and clinical leader to members of the nursing team
    • Offer and provide clinical supervision and support to members of the nursing team.
    • Share professional knowledge and experience with colleagues and health education providers to facilitate the provision of appropriate health care education.
    • Promote the development of a multidisciplinary approach to health care.

    Chester Central PCNis a collaboration between six GP practices across central Chester with a combined population of 38,962.

    We have a number of additional roles in our PCN, including social prescribing link workers, paramedics, well-being coordinators and pharmacy teams

    Job responsibilities

    • Initiate and participate in clinical audit, encourage a PCN-wide approach across the whole team, alerting colleagues to findings and the implication of those findings.
    • Evaluate the outcomes of clinical audit, applying findings to own and the teams practice, leading improvements in practice as the result of audit.
    • Contribute to the evaluation of relevant research findings to further develop the practice services.
    • Identify key areas for research/audit; develop appropriate methodology and supervise research outcomes and analysis of findings.
