Description
Responsible to: PCN and Practice Management Teams.
Overall
Objective- To provide support and assistance across the PCN and Practice
management teams with an emphasis on designing and implementing strategies to
enhance patient and staff digital experience.

Specific
areas of responsibility:
Digital
Transformation/Projects
-
Working with GP Partners, Practice
and PCN management teams, create strategies to encourage adoption of new
technological services and products which enhance the patients experience of
primary care alongside increasing efficiency and improving patient safety.

These projects will change on a rolling
basis fitting with the current PCN and Practice strategy.

Attend and contribute to regular
PCN and Practice meetings as requested.
-
Organise and conduct
training for new systems.
-
Proactively investigate new
systems and processes to streamline areas of working.
-
Work with the Practices on internal
service improvements and process changes.
-
Support identification, evaluation,
and ongoing reviews of current systems including producing data/reports.
-
Understand
software such as AccuRx and E-consultations in order to train others and
provide data/searches as needed.

Understand
EMIS admin functions such as building templates and concepts.
Patient education and Practice Communication
-
Distribute appropriate external information- directing
information efficiently to the correct person, agenda or team.

Initiate updates and content of Practice and
PCN Website.

Manage and make full use of Social media platforms.

Create resources for patient education and
practice communication.
General Practice and PCN Support
-
Evaluate projects with impact data, with a focus on quality
improvement outcomes.
-
This will include interpreting feedback and identifying
improvements for the PCN Virtual Group Consultation programme.
-
Identify priorities for PCN and Practices through
analysis of population health management data.
-
Support Practices with extracting and interpreting data
through a variety of mechanisms, including telephony data and Apex.
-
Oversee supply and repair of laptops for ARRS and PCN
staff.
Personal Development:
The Digital and Transformation Lead will:
Undertake
training needed to ensure competencies for delivering all responsibilities.
Attend
and contribute to in-house training events.
Take
responsibility for own development including giving and receiving feedback.
Undertake
regular mandatory training (child protection, vulnerable adults, basic life
support, etc.)
Confidentiality:
In the course of their duties, the Digital and
Transformation Lead will have access to confidential information relating to
patients, their carers and relatives and colleagues within and outside the PCN
and Practices. They may also have access to information relating to the PCN and
Practices as a business. All such information from any source whatsoever will
be treated as strictly confidential.
Information relating to patients, their carers and relatives
and colleagues within and outside the PCN and Practices and in relation to the PCN
and Practices as a business will only be shared in accordance with the PCN and
Practices Confidentiality Policy, the Data Protection Act and the Freedom of
Information Act and in such a way that personal and sensitive patient-identifiable
data is protected.

The Digital and Transformation Lead will be aware of and
comply with the Practices Health and Safety Policy. This will include:

The Digital and Transformation Lead may, from time to time,
be asked to undertake other reasonable duties, with appropriate training. It is
important that all members of staff are prepared to undertake additional or
relinquish current duties to maintain service delivery to patients. It is
expected that requests from the Partners/ Managers will not be unreasonably
refused.
