Job summary
Are you a proactive Independent Prescriber looking to transform patient care at the heart of our community? We are seeking a dedicated Clinical Pharmacist to lead medicines optimisation and provide expert clinical oversight across our network.
Main duties of the job
Key Responsibilities
Complex Clinical Reviews:Conduct comprehensive, person-centered medication reviews for patients with chronic diseases and complex polypharmacy. You will be the clinical lead for our most vulnerable populations, including the elderly and those residing in care homes.
Specialist Liaison & High-Risk Monitoring:Take a central role in managing shared care protocols. You will collaborate closely with other healthcare professionals to monitor and reduce the inappropriate use of high-risk medicines, ensuring patient safety is paramount.
System Integration:Act as a bridge between general practice and the wider healthcare ecosystem.
Workload & Outcomes Management:Proactively manage GP workload by leading on medicines-related queries and self-care initiatives, focusing on reducing medicine waste and improving long-term patient outcomes.
Expertise:A deep understanding of medicines optimisation and the ability to apply clinical evidence to real-world, multi-morbid patient scenarios.
Collaborative Spirit:The ability to work across multidisciplinary teams (MDTs) to integrate services and provide "joined-up" healthcare.
Patient Focus:A commitment to the "shared decision-making" model, empowering patients to take an active role in their own health.
