Job summary
Sevenfields Primary Care Network (PCN) is seeking a motivated and experienced Paramedic to join our multidisciplinary team under the Additional Roles Reimbursement Scheme (ARRS). The successful candidate will play a key role within our Home Visiting Service and provide Quality and Outcomes Framework (QoF) support across our four practices, serving a combined population of approximately 41,000 patients.
Working closely with GPs, nurses, and the wider primary care team, you will help deliver high-quality, patient-centered care to housebound and vulnerable individuals, supporting continuity of care and proactive management of long-term conditions. You will be accountable to Sevenfields PCN and will collaborate across practices to ensure service flexibility, resilience, and effective cross-cover.
This role plays a vital part in supporting our GP practices by carrying out home visits, helping to reduce hospital admissions, and enhancing patient outcomes through the delivery of timely and effective healthcare in the home setting.
Main duties of the job
Work in partnership with GP practices to deliver high-quality, responsive healthcare services to patients in their homes, including comprehensive assessment, diagnosis, treatment, and follow-up care.
Collaborate closely with GPs, nurses, and other health and social care professionals to ensure coordinated, holistic patient management.
Conduct patient assessments, take detailed medical histories, and perform thorough physical examinations during home visits and surgery consultations.
Identify, analyse, and diagnose medical conditions, recommending and explaining appropriate diagnostic tests and treatment options.
Undertake specialised diagnostic examinations and treatment procedures within professional competence and clinical governance frameworks.
Order and interpret laboratory and diagnostic tests as required and agreed under the supervision of the GP or clinical lead.
Formulate evidence-based treatment and management plans, seeking guidance or referring to supervising doctors when appropriate.
Provide patient education and advice on health conditions, treatment plans, and preventive care to encourage self-management and support long-term wellbeing.
Ensure accurate and timely documentation of all clinical activities and patient interactions, meeting legal, professional, and administrative standards.
Contribute to reducing hospital admissions and improving patient outcomes through timely, effective care delivered in the home environment.
About us
Sevenfields PCN is embedded into its community with active link workers and a staff base at Goldsmiths Community Centre which also offers vaccinations and other services. We have a real passion for the health and wellbeing of our residents and have invested in four outdoor gyms for our local communities using surpluses generated by our vaccination work which was largely delivered by our active volunteer team. Our local population is somewhat older than other parts of Lewisham and contains several large care homes. It includes areas of extreme socioeconomic deprivation and the PCN is keen to find innovative ways of addressing the health challenges that accompany this. All of the GP practices in Sevenfields are shareholders (members) of our PCN Company which has a Board that provides clear leadership through strong GP practice representation. As is the case elsewhere, Primary Care in Lewisham is under pressure. The challenge for Sevenfields PCN is to work in partnership with our local practices, GP Federation and Integrated Care Board (at South East London and Place levels) across our neighbourhood to help shape and implement the transformation of Care in our locality ensuring that it can both survive and thrive while still contributing to the delivery of new models of care that are self-sustaining financially.

- 5 years experience post-registration.
- Full HCPC registration as a Paramedic.
- Valid UK driving licence with no more than 4 penalty points.

- Level 7 modules in Advanced Practice, including Advanced Assessment.
- Experience as a clinical mentor.
- Previous experience in primary care or community-based healthcare.
