Job summary

Sevenfields PCN is looking to employ an Advanced Nurse Practitioner (qualified to Advanced Care Practitioner level) to support the our Primary Care Network team in the delivery of nursing services, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. You should be an enthusiastic and highly motivated ANP ideally with GP Practice background experience to work at the PCN level. The role will involve working three days a week at Oakview Family Practice as well as varied work within our PCN. The successful candidate will be using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base in line with the latest RCN guidance on ANP competencies.

Main duties of the job

The Advanced Nurse Practitioner will work within their professional boundaries

  • Providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
  • To undertake surgeries working in an autonomous role including the management of minor ailments, chronic disease, family planning, home visits to include both acute visits and the management of chronic medical problems in the house-bound
  • To provide telephone based triage, assessments and consultations.
  • Demonstrate safe, clinical decision-making and expert care for patients within the general practice maintaining clinical excellence.
  • Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.
  • Develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of PCN Patient population
  • Promote education and training to other staff and students, acting as a training resource
  • The post holder will work strictly in accordance with PCN protocols, policies and guidelines after appropriate training and assessment of competences have been completed and satisfied. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop

About us

Sevenfields Primary Care Network (PCN) is one of the largest PCNs (of six) in Lewisham and we are seeking to be a key driving force for improving community health and wellbeing in our neighbourhood. Our PCN is made up of seven GP Practices serving a population of around 70,000, one of which is Oakview Family Practice where our Clinical Director Dr Helen Tattersfield is based. We are seeking an Advanced Norse Practitioner (Advanced Care Practitioner) to support Oakview Family Practice and our local PCN.

Our local population is somewhat older than other parts of Lewisham and contains several large care homes. It includes areas of extreme socioeconomic deprivation and the PCN is keen to find innovative ways of addressing the health challenges that accompany this. Sevenfields PCN has created a company to host its staff and work and all of the GP practices in Sevenfields are shareholders (members), sitting on our PCN Board that provides clear leadership through strong GP practice representation.

Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan

Participate in developing and delivering specialist primary care services such as long term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).

Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered

Work with multi-disciplinary teams within the practice and across the wider CIC, to promote integrated and seamless pathways of care

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery

Participate in identification of community health needs and develop patient/family-centeredstrategies to address them in liaison with appropriate community health professionals and agencies

Be aware of and contribute to, planning and delivery of practice-based commissioning
