Job summary

We are a large Primary Care Network of 16 practices and are bringing our Social Prescribers, Health & Wellbeing Coach and Care Coordinators (Personalised Care Team) in house from the 1st April 2024.

For this reason, we have created a new role to support the team developing the in-house service provision between now and the end of March 2024 and to then provide Administrative support to the Team Leaders moving forwards.

Main duties of the job

You will be involved with the implementation of a new computer system (Joy) to help us support the Personalised Care Team, manage patient referrals and monitor Lone Working for the staff so need to be good at communicating and enjoy working as part of a team.

Some of the services that we provide for our patients include a 'Walk and Talk' initiative and 'Healthy Lifestyles' sessions. Part of your role will be to work with the Health & Wellbeing Coach to coordinate patients numbers and support for the group meetings.

The Personalised Care Team meets every two months to share information and provide peer support/case reviews. You will work with the Team Leaders to organise these meetings, send out invitations, create agendas and produce the minutes.

An exciting opportunity has arisen to join the dynamic and growing team at Nuneaton and Bedworth Primary Care Network (PCN). This role will work as part of the Personalised Care Team to provide administrative support in terms of processing referrals into the team, maintaining the referrals/marketplace database and supporting the Senior Team Members.

Job Purpose/Summary

Our Social Prescribers, Health & Wellbeing Coaches and Care Coordinators are currently employed by Third Party organisations, and will be transferring into the employment of the PCN from the 1st April 2024. For this reason, we are working with the teams to develop our own systems in-house, so this is a brand new role to support the teams and work with us to develop those systems.

We have recently procured some new software called Joy and are in the process of establishing a project team to ensure a timely and effective implementation. You will be part of this team to support a smooth transition of service provision in-house.

As this is a new position, it is anticipated that the role will evolve and grow, but key responsibilities will include:

Provide Administrative support for the Personalised Care Team (Social Prescribers, Care Coordinators, Health & Wellbeing Coaches and Mental Health Practitioners). This will include coordinating and taking minutes for the bi-monthly MDT Meetings.

Monitor and allocate referrals coming into the Team via the Joy system.

Support Lone Working arrangements for members of the team when undertaking off-site visits to see patients.

Work with the Health & Wellbeing Coach(s) to support the Walk & Talk and Healthy Lifestyles projects.

Maintain the Marketplace within the Joy system to ensure that organisation details remain up to date for referral pathways for patients.

Induct new starters in use of the Joy system.
