Job summary
We are looking to recruit an enthusiastic and highly
motivated Practice Nurse to join our small friendly and supportive team in a
4000 list SystmOne practice.
Ideally, we are looking for someone who has previous experience
in Primary Care and who is trained in dealing with Chronic Disease Management,
Cervical Screening and Childhood Immunisations.
We are looking to cover approx. 30 - 35 hrs but would be
willing to consider full or part-time for the right candidate.
The post holder will be an experienced nurse acting within
their professional boundaries.
You will work collaboratively with the general practice
team to meet the needs of patients, supporting the delivery of policy and
procedures
You will be responsible for the clinical nursing care of
the practice population. You will receive referrals from the general
practitioner and nurse practitioner and will be called upon to advise, support
and treat patients who attend the surgery. You will undertake the delivery of
nursing care in general practice as a self directing practitioner, working with
the GPs to meet patient needs as appropriate.
Main duties of the job
- Cytology
- Wound care / ulcer care/Doppler
- Removal of Sutures / post operative care
- Urinalysis
- ECGs
- Chronic disease management in line with QOF requirements
- Venepuncture
- Hypertension Management
- Ear syringing
- Routine immunisations/Childhood immunisations
- Chaperoning and assisting patients where appropriate who
are being examined by another clinician
- Requesting pathology tests, for example urine culture,
swabs
- Following agreed clinical protocols with referral to GPs as
appropriate
- Other tasks will include
- Maintaining GP and Nurses rooms, stocking and rotating
items as required
- Maintain general tidiness and cleanliness of nurses and
treatment rooms
- Ordering of Health Promotion Literature
- Ordering of stock
- Ordering of vaccinations to maintain stock levels
- Maintain and embed robust infection control measures.
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate
records
- A duty to advise the Practice Manager of potential problems
or errors within the range of assigned tasks
- Attend and participate in any Practice meetings when
required.
- On occasion there may be a requirement to undertake home
visits

aspects of treatment room nursing and have the ability to work within a team.
Your skills should also include, but are not limited to,
cervical screening, wound care, childhood and other vaccinations. In addition,
qualifications and expertise in chronic disease management is advantageous.

PCN we offer enhanced access, and attached staff including, physiotherapist,
podiatrist, health and wellbeing coach, clinical pharmacist, care coordinator,
mental health support and social prescribers. Everyone within the team and wider
