We are currently seeking an Independent Prescribing Pharmacist to join our friendly, multidisciplinary team. This is a fantastic opportunity to contribute directly to patient care, leading on safe and effective prescribing, managing chronic conditions, and supporting medication optimisation across our practices.

Main duties of the job

Conduct patient consultations and prescribe within your clinical competence Perform medication reviews and support deprescribing initiatives Collaborate closely with GPs, AHPs, nurses, and other professionals to enhance care pathways Take a proactive role in managing long-term conditions and promoting antimicrobial stewardship Provide training and guidance to colleagues on medicines-related best practice

About the Role:

We are looking for a highly skilled GP Independent Prescribing Pharmacist to join our dynamic primary care team. This is an exciting opportunity to work within a GP practice, contributing to patient care by providing high-quality prescribing and medication management services.

Key Responsibilities:

Undertake patient consultations, assessments, and prescribing within your clinical competence.

Provide expert advice on medications, ensuring safe and effective prescribing in line with local and national guidelines.

Conduct medication reviews, particularly for patients with long-term conditions.

Support the management of chronic diseases such as diabetes, asthma, and hypertension.

Work collaboratively with GPs, nurses, and other healthcare professionals to optimise patient care.

Deliver structured medication reviews and deprescribing initiatives where appropriate.

Support antimicrobial stewardship and contribute to the reduction of inappropriate prescribing.

Participate in audits and quality improvement initiatives within the practice.

Provide training and support to practice staff on prescribing and medication-related issues.

Essential Requirements:

Registered Pharmacist with the General Pharmaceutical Council.

Independent Prescriber qualification.

Experience in primary care or general practice settings.

Strong clinical decision-making and problem-solving skills.

Ability to work autonomously as well as part of a multidisciplinary team.

Excellent communication and patient consultation skills.

Desirable Skills & Experience:

Experience in managing long-term conditions.

Knowledge of GP systems such as EMIS or SystmOne.

Experience in conducting medication reviews and structured prescribing interventions.
