Job summary

Elswick Family Practice is currently recruiting for a Salaried GP to join our team for 4 sessions per week. This is an excellent opportunity to be part of a supportive, values-driven practice committed to high-quality care and professional development. Salary is dependent on experience.

We are a recognised Deep End practice,actively working to address the health inequalities faced by patients in areas of high socioeconomic deprivation. Our team is passionate about equitable access to care, social justice in healthcare, and supporting vulnerable communities through integrated, compassionate services.

Our wider team includes practice nurses, healthcare assistants, PCN pharmacists and first-contact practitioners, and a dedicated administrative staff who work closely together to deliver integrated, patient-focused care.

We use SystmOne clinical system and are fully digital, with efficient systems supporting eConsult, electronic prescribing (EPS), and document workflow. As an active member of our local Primary Care Network (PCN), we engage in collaborative health initiatives and enhanced services that benefit both patients and staff.

Main duties of the job

You will be expected to provide high-quality, patient-centred primary care to a diverse population in a deprived area as part of a Deep End practice.

Undertake routine and urgent consultations, chronic disease management, and preventive health measures.

Participate in multidisciplinary team meetings to deliver holistic care, with a focus on reducing health inequalities.

Contribute to care for frail patients, including care home ward rounds and proactive management of complex needs.

Support teaching and supervision of medical students or trainees (advantageous but not essential).

Engage in quality improvement activities and practice development initiatives.

Job Summary:

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical Responsibilities:

  • Will be available to undertake a variety of duties including surgery consultations, telephone consultations and queries, home visits, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
  • Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
  • Assess the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with practice disease management protocols, developing care plans for health.
  • Provide counselling and health education
  • Admit or discharge patients to and from the caseload and referring to other care providers as appropriate
  • Record clear and contemporaneous consultation notes to agreed standards.
  • Collect data for audit purposes
  • Compile and issue computer-generated acute and repeat prescriptions
  • Prescribe in accordance with Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
