Job summary
Ramsbottom Medical Practice is a well-established,
forward-thinking GP practice serving approximately 8200 patients. We work as a
multidisciplinary team and are committed to safe, high-quality patient care,
good governance, and supporting staff wellbeing.
We are seeking an experienced Advanced Nurse Practitioner
to join our clinical team for 3 days per week. This is a permanent position
of 24 hours per week Monday, Wednesday and Thursday (these days are non-negotiable).
Salary is negotiable depending on experience.
Main duties of the job
Independent patient assessment, diagnosis, and
treatment
Management of acute, urgent, and on-the-day
presentations
Assess and triage patients, including same day
triage, and as appropriate provide definitive treatment
Chronic disease management
Prescribing in line with qualification and local
guidelines
Appropriate referral, escalation, and
safety-netting
Accurate and timely clinical documentation
Working collaboratively with GPs, nurses,
pharmacists, and the wider MDT
There may be, on occasion, a requirement to
carry out other tasks, this will be dependent upon factors such as workload and
staffing levels
Maintain accurate clinical records in
conjunction with extant legislation and to ensure SNOMED CT codes are used
effectively
Engage in robust governance systems and
contribute to practice targets QOF, etc., complying with local and regional
guidance

Our clinical team includes GP Partners, salaried GPs, Practice Nurse, Healthcare Assistant, Clinical Pharmacists and a wider PCN team, working together with clear clinical governance and mutual support. The practice is underpinned by a strong and experienced management and administrative team, including reception, care navigation, secretarial and operational staff, who play a vital role in keeping services running smoothly and allowing clinicians to focus on patient care.

The Advanced Nurse Practitioner ANP will work autonomously within their agreed scope of practice to provide high-quality, patient-centred clinical care in a general practice setting. The post holder will assess, diagnose, treat, and manage patients presenting with acute, urgent, and chronic conditions, ensuring safe clinical decision-making, appropriate escalation, and robust documentation.
