Job summary
Working as part of a multi-disciplinary team in a patient-facing role,
you will support and work alongside a team of GPs in general practice,
providing primary support to general practice staff with regard to prescription
and medication queries. You will help support the repeat prescription system,
deal with acute prescription requests, and undertake clinical medication
reviews to proactively manage patients with complex polypharmacy. Your role is
pivotal to improving the quality of care and operational efficiencies so
requires motivation and passion to deliver excellent service within general
practice. You will be supported by a General Practitioner who will develop,
manage and mentor you while working at Ramsbottom Medical Practice.

and continuous professional development are integral to both the role and the

Main duties of the job
Ideally you will be an independent prescriber
Conduct comprehensive medication reviews.
Manage repeat prescriptions and ensure safe and effective use of
medicines.
Deal with acute prescription requests.
Medicines reconciliation on transfer
of care.
Undertake structure clinical medication reviews with patients and
produce recommendations on prescribing and monitoring.
Patient Consultations:Provide expert advice and
support to patients regarding their medications.
Clinical Services:Support the management of
long-term conditions and participate in chronic disease management clinics,
based on evidence and practice protocols when available.
Prescribing:Suggest medications within the scope of
practice and in accordance with local and national guidelines.
Collaboration:Work closely with GPs, nurses, and other
healthcare professionals to optimise patient care and improve clinical
outcomes.
Education and Training:
To maintain professional and clinical competence through
mechanisms of continuing professional development and staying up-to-date with
the latest clinical guidelines.
Participate in clinical audits of prescribing in areas directed by
the GPs and quality improvement initiatives. Feedback on the results and
implement changes in conjunction with the practice team.

Ramsbottom Group Practice is a training practice based in
the centre on Ramsbottom serving a practice population of approximately 8,300
patients.
We are a friendly team consisting of 3 Partners, 2 salaried
GPs, a nursing team, along with our admin and reception team.
We are looking to appoint a highly valued and independent
pharmacist working within our primary care team with active contribution to
achieving the highest standards of patient-centred care.

Medication Management:
Conduct comprehensive medication reviews.

Reconcile medicines following discharge from
hospitals, intermediate care and into care homes, including identifying and
rectifying unexplained changes, working with patients and community pharmacists
to ensure patients receive the medicines they need post-discharge, and setting
up systems to ensure continuity of medicine supply to high-risk groups of
patients.
Produce and implement a practice repeat prescribing policy,
managing the repeat prescribing reauthorisation process by reviewing
patient requests for repeat prescriptions and reviewing medicines reaching
review dates and flagging up those needing a review, ensuring that patients
have appropriate monitoring tests in place when required.
Identification of cohorts of patients at high risk of harm from
medicines through pre-prepared practice computer searches. This might
include risks that are patient related, medicine related, or both.
Monitor practice prescribing against
the local health economys RAG list and make recommendations to GPs for
medicines that should be prescribed by hospital doctors (red drugs) or subject
to shared care (amber drugs).
Liaise with community and hospital
pharmacies to help utilise skill mix, improve patient outcomes, ensure better
access to healthcare and help manage workload.
