We are looking for an enthusiastic and committed ANP to join our friendly and supportive team who are passionate about delivering high quality health care.
Main duties of the job
The role of a Nurse Practitioner within the primary care setting is to practice at an advanced level, making professionally autonomous decisions for which he/she holds sole responsibility.
The Nurse Practitioner will attend to people with undifferentiated and undiagnosed problems. Responsibilities will include nurse triage, assessment and treatment, screening and patient education.
The purpose of this post is to provide a comprehensive triage service throughout the working day and to provide patients with direct access to primary care services.
The role aims to develop a balanced, skilled, practice nurse team, hence enhancing overall patient care.
About us
Our Practice list size is approximately 11,400 patients with a core team of 9 GPs, 1 long term locum GP, ANPs, established nursing team, well-staffed reception and admin; wider team including Clinical Pharmacists, Social Prescribers, Health & Wellbeing Coach, Physiotherapist.
We are a dispensing practice with two sites - patients can be seen at either site and staff work across both sites.
We belong to a progressive local GP network, Bosworth PCN, within a GP Federation, Hinckley & Bosworth Medical Alliance

MAJOR DUTIES
A Clinical Practice
- To be first point of contact for patients requesting contact with Primary Care for specified conditions, to carry out a full health assessment of their health care needs, based on highly developed nursing knowledge and skills. To provide advice, initiate treatment (including prescribing) and make referrals as appropriate.
- To be competent in dealing with general nursing care of the patient within the surgery. Examples of this include wound care, ear syringing, eye washing, venepuncture, electrocardiography, management of skin disorders, recognising mild childhood illnesses and giving immunisation and vaccinations.
- Assist in the provision of care to those patients with long term conditions operating specific disease clinics, for example asthma, diabetes, coronary heart disease.
- To screen patients for disease risk factors, and early diagnosis of illness and to help monitor a recall system for patients.
- Refer to other primary, community and secondary care members within locally developed pathways.
- To work in a partnership with general practitioners, practice nurses and other professionals to develop protocols, standards, and audit tools to facilitate effective delivery of care.
B Administrative and Professional Responsibilities
Participate in the administrative and professional responsibilities of the practice team.
- Involvement in policy developments for clinical issues with the practice, providing the nursing input to any decision-making processes affecting the care of patient.
- To foster multi-disciplinary and multi-agency working in the development of practice initiatives.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
C Education, Training and Professional Development
Work within the latest NMC Code of Professional Conduct.
Maintain their professional registration.
Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of revalidation, audit, and peer review.
Participate in continuing professional development opportunities (attending necessary courses & study days) to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.
Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
Work collaboratively with colleagues within and external to the practice.
Pro-actively promote the role of the ANP within the Practice and externally to patients, key stakeholders, and agencies.

D Health Promotion and Disease Prevention
To target health promotion activities according to the epidemiology/health needs of the practice population and the area indicated by the Health Improvement Programme.
To be involved in the organisation of health promotion and chronic disease management programmes within the practice.
To manage and audit the national and local screening programmes with the practice population.
To initiate a Nursing case load of patients that are appropriate to be managed within nurse led clinics.
E Communication
Provides and receives highly complex, sensitive, or contentious information and occasionally deals with situations which are hostile or emotive.
Uses developed communication, negotiation, conflict management skills which require empathy, reassurance, and persuasive skills where there can be significant barriers to acceptance which need to be overcome.
Regularly communicates service-related information to the Practice Manager, GPs, Nursing and Administrative staff, patients, relatives, and carers.
Makes operational judgements, manages conflicting views, and reconciles inter and intra professional differences of opinion.
Communicate effectively with other members of the practice, patients, and carers.
Recognise peoples needs for alternative methods of communication and respond accordingly.
F Manage/Lead Self/Others
Develops a special interest (dependent on experience) within the practice (e.g. teenage clinic, baby clinic, nursing home, cytology).
Encourage and motivate others to be forward thinking in ideas that address the health needs of the practice that will be innovative whilst managing and developing new ways of working, (e.g. Implanon clinic, teenage health clinic).
Develops and leads on evidence-based practice.
Promotes effective communication and relationships within the team.
Partakes in monthly clinical supervision for own development.
Actively participates in clinical and practice meetings.
Develops own knowledge and practice to meet objectives/changes in service, through attendance on study days, self-directed learning, and reflection on practice.
Integrates theory into practice and establishes formal links with educational institutions.
Assists in the mentorship and assessment of trainee Nurse Practitioners.
Acts as mentor for pre-registration students and other staff within the practice.
Leads on setting standards and promotes best practice.

F Manage and Use Information
Makes effective use of the clinical system by ensuring the consistent and accurate entering of data and use of Read Codes.
Facilitates the use of up-to-date information technology in order to collate accurate and timely information as and when required by the practice.
Ensures a satisfactory call and recall system of patients.
Assist in the development of practice protocols for the surgery and clinical protocols for the clinical team.
Identifies monitors and reviews deficiencies in procedures and implements remedial action.
H Working Conditions
Frequent exposure to body fluids, blood, and wounds.
