Job summary

We are looking to add one further Clinical Pharmacist to our expanding PCN MDT Team to work across Maidenhead Practices in delivering patient-centred care by:

Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.; Completing the Clinical Pharmacist in Primary Care Pathway and become a prescriber to support the Practice Team; Be responsible for the pro-active care management of patients with complex polypharmacy; Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs of patients and tackle health inequalities; Provide leadership on person-centred medicines optimisation and quality improvement.

Main duties of the job

Staff undertaking these roles will

See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma); Undertake structured medication reviews with patients and produce recommendations for agreement on prescribing changes and monitoring; Work with care home staff to improve safety of medicines ordering and administration; Provide patient facing clinics for those with questions, queries and concerns about their medicines in practices; Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews; reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

Maidenhead Primary Care Network, is a group of 9 GP Practices working together with a range of local providers to offer more personalised and coordinated health and social care to the 70000 patient population of Maidenhead in Berkshire. While each Pharmacist is embedded in up to 2 Practices, they frequently enjoy a mutually supportive joint approach to work with other members of the 5 strong PCN Pharmacist Team. The Team benefit from both practice and PCN Clinical supervision and are supported with study leave to complete the CPPE Clinical Pharmacist in Primary Care Pathway and to become Prescribers.

Job description

Job responsibilities

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinician and Line Manager who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role.

The post holder will take responsibility for areas of chronic disease management within PCN practices and undertake Structured medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in GP practice (s).

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practices integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Essential

  • Mandatory registration with General Pharmaceutical Council Masters degree in pharmacy (MPharm) (or equivalent)

Desirable

  • Membership of the Royal Pharmaceutical Society A member of or working towards Faculty membership of the Royal Pharmaceutical Society Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience Independent prescriber or working towards/intent of gaining independent prescribing qualification
