Job responsibilities
Holistically
bring together all a persons identified care and support needs and explore
options to meet these within a single personalised care and support plan
(PCSP) in line with PCSP best practice based on what matters to the person.
Help
people to manage their needs through answering queries making and managing
appointments and ensuring that people have good quality written or verbal
information to help them make choices about their care.
Support
people to take up training and employment and to access appropriate benefits
where eligible.
Support
PCN staff and patients to understand their level of knowledge skills and
confidence (there Activation level) when engaging with their health and
wellbeing including using the Patient Activation Measure.
Assist
people to access self management education courses peer support or
intervention that support them in their health and wellbeing and increase their
activation level.
Explore
and assist people to access personal health budgets where appropriate.
Provide
coordination and navigation for people and their carers across health and care
services working closely with social prescribing link workers health and
wellbeing coaches and other primary care professionals.
Support
the coordination and delivery of MDTs within the PCN.
In addition to the primary responsibilities the
care coordinator may be requested to
Work with the
GPs and other primary care professionals within the PCN to identify and manage
a caseload of patients and where required as appropriate refer to other
health professionals within the PCN.
Raise
awareness within the PCN of shared decision making and decision support tools.
Demonstrate a
flexible attitude and be prepared to carry out other duties as may be
reasonably required from time to time within the general character of the post
or the level of responsibility of the role, ensuring that work is delivered in
a timely and effective manner.

Confidentiality and
Information Governance
Island City Network is committed
to maintaining an outstanding confidential service. Patients entrust and permit
us to collect and retain sensitive information relating to their health and
other matters pertaining to their care. They do so in confidence and have a
right to expect that all staff will respect their privacy and maintain
confidentiality. It is essential that if the legal requirements are to be met
and the trust of our patients is to be retained, all staff protect patient
information and provide a confidential service.
Island City Network is
registered under the Data Protection Act 1984. You will have to comply with the
Data Protection Act and the Access to Health Records Act the General Data
Protection Regulations 2018, and any network policies regarding information governance
at all times. You must not at any time use the personal data held by the
practice for a purpose not described in the register entry or disclose such
data to a third party.
Quality and
Continuous Improvement (CI)
To preserve and improve the
quality of our output all personnel are required to think not only of what
they do but how they achieve it. By continually re-examining our processes we
will be able to develop and improve the overall effectiveness of the way we
work. The responsibility for this rests with everyone working within the
network to look for opportunities to improve quality and share good practice.
Island City Network continues to
strive to improve work processes which deliver healthcare with improved results
across all areas of our service provision. We promote a culture of continuous
improvement where everyone counts and staff are permitted to make suggestions
and contributions to improve our service delivery and enhance patient care.
Induction training
At Island City Network you will
be required to complete the induction programme this is managed by your line
manager.
Learning and
development
The effective use of training
and development is fundamental in ensuring that all staff are equipped with the
appropriate skills knowledge attitude and competences to perform their role.
All staff will be required to participate in and complete mandatory training as directed by your line manager as well as participating in the networks training
programme. Staff will also be permitted (subject to approval) to undertake
external training courses which will enhance their knowledge and skills,
progress their career and, ultimately enable them to improve processes and
services delivery.
Collaborative working
All staff are to recognize the
significance of collaborative working. Teamwork is essential in
multidisciplinary environments. Effective communication is essential, and all
staff must ensure that they communicate in a manner which enables the sharing
of information in an appropriate manner.
Managing information
All staff should use technology and appropriate
software as an aid to management in the planning, implementation and monitoring
of care and presenting and communicatinginformation. Data should be
reviewed and processed using accurate SNOMED/read codes to ensure easy and
accurate information retrieval for monitoring and audit processes.
Service delivery
Staff at Island City Network
must adhere to the information contained within the networks policies and
regional directives, ensuring that protocols are always adhered to. Staff will
be given detailed information during the induction process regarding policy and
procedure.
Security
The security of the network is
the responsibility of all personnel. Staff must ensure that they always remain
vigilant and report any suspicious activity immediately to their line manager.
Under no circumstances are staff to share codes for the door locks with anyone
and they are to ensure that restricted areas remain effectively secured.
Professional conduct
At Island City Network, staff
are required to dress appropriately for their role.
